Tiburcio Vasquez Health Center is a nonprofit CHC dedicated to promotion the health and wellbeing of our community by providing accessible, high-quality care to its patients. This clinical training program is a one-year paid internship which aims to bridge the skill gaps of new NP/PA graduates to enable smooth transition to a NP/PA role. New NPs often struggle to keep up with job expectations because of lack of clinical training. This program will help build proficiency and efficiency demanded in employment settings and provide an opportunity for new grads to understand our work culture, patient population. This is a full-time benefited position working 40 hours per week, typically Monday through Friday with periodic Saturday hours. Program outlineDuration is up to 1 year which is divided into 3 Tiers:First Tier: Administration of medications and immunizations to patientsHealth instruction/ Education to patientsMedication reconciliation Referrals to community health resourcesTriage patients, comprehensive history taking, examination and documentation Supervise clinic workflow and assist providers and MA when needed. Organize orientation, training, growth and development of LVN and MA staffMaintain inventory of medical supplies and reorder as necessary Assist providers with tasks delegated Attend general and medical staff meetings and participates in quality improvement projects.Assist the Chief Medical Officer and COO on matters relating to nursing practices and participates in development of policies and protocols for patient care. Licensed and credentialed to practice as a NP providerSecond Tier: Inbox coverageManaging low acuity patients; UTI, Immunizations, Depo Shots, Medication refills, Sore throat, Gastroenteritis, high BP, asthma exacerbation, vaginitis, joint pains, annual physicalsPatient education on DM, tobacco cessation, obesityAnxiety and depression management and linking to BHSeeing new patients to TVHCPlacing referrals for acute ER visits (ortho / ophthalmology)Shadow/ Precept and scribe to grasp EPIC workflow. ½ day per week with PreceptorPanel building startsThird Tier: Continue nursing duties and previous quarter expectationsManage chronic conditions like COPD, CHF, CKD, DM, Thyroid disordersTo be able to manage a complex patient with multiple conditionsTo be able to manage time with increasing panel size, goal of 18 patients at the end of this tierAble to use cost effective and evidence-based diagnostics and referrals.Timely documentation in EPIC of patient visits, management of inbox and paperwork completionReferrals to specialist as needed for complex and high priority patient cases.
