For more information, go to , and interact with us on Facebook, Twitter, YouTube and Instagram.Position Summary:We have an exciting opportunity to join our team as a RN this role, the successful candidate as a Nurse Navigator based at our PCC Sunset Park location. The Nurse Navigators primary focus is to ensure that patients receive timely and appropriate navigation to diagnostic and treatment services and that continuity of care is maintained. In this role, the successful candidate is responsible for coordinating, implementing, and evaluating the treatment plans for PCC Sunset Park thoracic oncology patients in conjunction with the attending physician, multidisciplinary team, patient and family. The Nurse Navigator will also provide patient and family education and information for therapeutic and non-therapeutic clinical trials and research studies for lung cancer screening and newly diagnosed thoracic oncology patients, as well as in community based settings targeted for the general public. The Navigator will work collaboratively with the Lung Cancer Screening Program team.Job Responsibilities:Works collaboratively with the interdisciplinary team including the attending physician to identify and implement an appropriate plan of care for Lung Cancer Center patients that reflects shared decision making and incorporates individual patient/family preference and enhances quality, access,and cost-effective outcomes.Facilitates the continuity of care across the continuum of services, e.g. home care, other facilities, MD appointments, etcEnsures orders are placed for laboratory, radiological and diagnostic studies per practice protocols, differentiates between normal/abnormal findings and follows protocols for abnormal findings with collaborating provider.Performs complete health history/ assessments and psychosocial assessment of patients within the designated practice or clinical unitFunctions as a respectful members of the healthcare teamParticipates in designated activities related to professional nursing standards and regulations of relevant outside agenciesWorks collaboratively with the attending physician and nursing leadership to maintain timely and appropriate communication with family members, house staff, other disciplines and ancillary servicesDemonstrates knowledge of the organizations. Service Standards and incorporates them into the performance of responsibilitiesParticipates in the development and revision of interdisciplinary and nursing standards, patient and family education materials as relevant to the PCC and PCCs Lung Cancer CenterArranges for medical equipment, devices, physical and occupational therapy as ordered, and home health services per practice protocol.Practices as a member of the allied health staff according to the rules and regulations of Nursing Practice.Assists in the preparation, implementation, and evaluation of research protocols when applicableWorks collaboratively with nursing and other disciplines in the development and implementation of clinical studies within the area of expertise.Participates in the development and monitoring of patient outcomes per established practice protocols for purposes of quality and performance improvementArranges for medical equipment, devices,physical and occupational therapy as ordered, and home health services per practice protocolPractices as a member of the allied health staff according to the rules and regulations of Nursing PracticeUses evidence-based health care literature to advise and support appropriate practice changes within the designated service.Serves as a clinical expert and resource for the education of peers and other health professionals.Maintains current expertise in area of practice.Promotes own professional growth and development in clinical role.Considers needs and behaviors of patients receiving lung cancer early detection and treatment services in Sunset Park and surrounding communitiesProvides community-based education.Minimum Qualifications:Education: Current registration as Professional Nurse in New York.Three to five (3-5) years working in ambulatory care setting.Proficiency in utilizing electronic medical records, Microsoft Office SuiteExcellent communication skillsGood clinical assessment skills and judgmentPatient advocateExperience: Creative problem solving and excellent critical thinking and leadershipCollaborative member of multidisciplinary teamPreferred Qualifications:At least 5 years of clinical nursing practice in oncology.Bilingual Spanish or MandarinKnowledge of lung cancer and/or clinical trials.Some public speaking.Experience facilitating group processesExperience in nurse navigation, case management or patient counseling and educationKnowledge of existing community based organizations in Sunset Park and surrounding neighborhoodsRequired Licenses: Registered Nurse License-NYSQualified candidates must be able to effectively communicate with all levels of the organization.NYU Langone Health provides its staff with far more than just a place to work.
