Erie Family Health Centers is looking for an addition to our Case Management Team! The Case Manager will work with medical providers and nursing staff to identify potential clients, assess, and deliver supportive services to clients.SummaryProvides case management services including: coordination of primary care (prenatal and/or pediatric care) and social services, outreach, face-to-face follow ups, risk assessments completion, referrals, home visitation, and track clients'' appointments failures. ResponsibilitiesOutreach, advocacy, assessments, care plan, home visitation, and EMR data entry.Collaborates with fellow Case Managers to provide constant floor coverage throughout the day.Displays familiarity and knowledge of the care plan of each client and facilitates referrals and follow-up processFollow the Department of Human Services Maternal and Child code requirementsMandated reporter to the Department of Children and Family ServicesCompletes Edinburgh post-partum screenings and Ages and Stages developmental screenings.Collects participant''s surveys as required by grantors and returns them to the designated point of collection within the department.Maintains quality of the cases by achieving Erie''s and grantor''s programs goals, including but not limited to, the following areas:Prenatal and Post-Partum Depression ScreeningsDevelopmental screeningsPrenatal face to face visitsInfants face to face visitsHome visitsEI referralsClients individual care plan and assessmentsPrimary care provider documentationFirst trimester enrollmentWell child visitsImmunizationsIntegration with WIC and case managementAny other program respective goalsDocument data into electronic systems (EMR & Cornerstone)Participates in annual audit of programsDistributes and maintains records of the clients'' transit cards, incentives, home visits, assessments, etc. Attends all required meetings, trainings and conferences regarding case managementPerforms other duties as assigned.RequirementsMSW required in social work, child development, community health, or related fieldMinimum of two years'' work experience in health, social or community services preferredBilingual (English/Spanish) required (May be required to assist or work with non-English/Spanish only speaking patients and/or guests)Local travel required, valid Illinois Drivers License and automobile insurance also requiredCurrent CPR required*COVID-19 Vaccination required for all Erie employees
