Tewksbury Hospital under the Department of Public Health (DPH), a 370 bed Joint Commission accredited hospital, is currently hiring for the position of RN III - DDS (Developmental Disability Services) Liaison Nurse - Patient Care Unit E4. The hospital provides a broad treatment of care and comfort to adults with medical and/or mental illnesses. Commonly performed duties include, but are not exclusive of the following: Provides direct care to patients between 20-100+ years of age with chronic medical and nursing needs. Assess, plan, implement and evaluate nursing care according to the needs of the individual patient or a group of patients, based on appropriate standards of practice including assisting in the development and maintenance of the individualized plan of care for individuals with intellectual disabilities. Reviews patient data to remain informed and knowledgeable about the dynamics of patient needs, strengths, abilities and potentials including having a knowledge base of all DDS clients. Develop a therapeutic relationship with DDS nurses. Collaborates and cooperates with all disciplines, gathering requested data from those members of the interdisciplinary team who are not present during meetings and other forums. Provides meaningful documentation including Patient Assessment, Plan of Care, Progress Notes, Discharge referrals and other required charting to maintain complete and accurate medical records including documenting communication with guardians, HCP, area office nurse, and other members of the DDS team. Accurately communicates information about patients to appropriate health care professionals (M.D., Social Service, Dietary; other nursing staff, etc.). Participates in patient care and unit conferences. Appropriately delegates, assists and supervises work assigned to other staff members. Participates in quality improvement projects with a focus on individuals with intellectual disabilities. Transcribes and appropriately carries out the physician''s orders for patients. Prepares appropriate assignments for nursing personnel based on patient needs and staff skills. Participates in change of shift patient reports. Accepts clinical assignments, hours of work, charge and staff responsibilities. Assists in the coordination and performance of education to be provided to community providers in preparation for discharge. Understands and initiates rules and regulations which govern fire, bomb threat and disaster. Serves on Nursing and Hospital committees as requested. Attends required educational seminars to maintain licensure, and other workshops and seminars to enhance own professional development. Attends required Inservice Programs annually. Compliance with all applicable state and federal laws including the Health Insurance Portability and Accountability Act (HIPAA) regulations which govern the privacy and confidentiality of information about patients. Attend all DDS meetings and provide report to unit staff about changes to plan of care. Assisting during discharge of all DDS clients including but not limited to preparing the discharge packet, ensuring all equipment is listed. Research all DDS requests as approved by the nurse manager. Assist with information gathering with concerns/complaints. Assist with sensory integration techniques. Assist with investigations as assigned. Assist Physician in aging and Developmental Disability Clinic. Process Intake Form. Maintain Medical Record. Oversee screening process and referrals. Performs related duties as required. MINIMUM ENTRANCE REQUIREMENTS: Applicants must have at least (A) one year of full-time or equivalent part-time, experience as a registered nurse in a recognized hospital, clinic or medical facility, and (B) six months of which must have been in a supervisory, administrative or managerial capacity.
