Position: Case Manager/Recovery CoachClassification: Part-Time, Per DiemCompensation:Bachelor''s Degree - $25-$45Master''s Degree - $45-$65PhD, PsyD - $65-$125Final rate of pay may vary from the above based on experience and skill set, and will be mutually agreed upon during the interview process.Summary: The case manager/recovery coach assists in clients'' care by developing, monitoring, and evaluating care plans and progress, coordinating an interdisciplinary team, and monitoring clients'' improvement. Case managers will coordinate care for clients with a range of behavioral health issues including substance use disorders, mental health issues, eating disorders, issues affecting those on the autism spectrum, mood disorders, and personality disorders. The case manager will act as a liaison between the client/family and treatment personnel to ensure prompt and effective care. The case manager will also communicate progress and observations to both OPG team members and non-OPG treatment providers. Case managers will meet clients in the community or home setting to perform actions necessary to meet their mental health, social, and financial goals.Job Responsibilities:Participating in initial consults and writing corresponding consult reports. Completing biopsychosocial assessments on clients.Reviewing past treatment records and contacting clinical providers.Coordinating and helping to complete a comprehensive consultation addressing the client''s educational, vocational, recreational, and social needs.Clinical: defining what clinical services will be accessed and how frequently. Facilitating efficient communication between treatment team, client, and family coach.Educational: assistance in selecting a school program. Supporting a client''s school plan, including class selection, connections, and school-based resources (i.e. tutoring and counseling).Vocational plan: assessment of vocational interests, short-term support for finding and keeping a job and long-term planning towards career goals.Recreational plan: discovery of clients'' interests in cultural, sports-related, and artistic pursuits. Connections to appropriate resources.Social: developing skills to pursue healthy behaviors and assistance in cultivating a positive support dependent living skills: creation and monitoring of a budget, assistance with time management and other basic living skills as needed.Creating short and long-term client goal plans; daily documenting of progress.Meeting with client 1-5 times per week in the community or home settings to monitor client progress, with follow up calls as appropriate.Providing connections to endorse community resources, treatment options, peer counselors and AA companions as appropriate.Providing observed in-home drug testing and Soberlink monitoring if appropriateEmergency support, on-call availability, and client transportation as needed.Additional duties will be assigned as deemed D100
