Bulfinch Temporary Service is hiring a temporary NP to assist from September 2022 - March 2023 to cover a leave of absence in Palliative Care and Geriatric Medicine. Please note this is a temporary position and is not benefits eligible but is a great way to get your foot in the door at MGH! This position is M-F from 8:30am-5pm. Under the general direction of Population Health Management for the hiring Regional Service Organization (RSO), the Geriatric Advanced Practice Nurse (APRN) is responsible for guiding the management of dementia care for adult primary care patients enrolled in the MGB Memory Care Initiative. Within the program''s collaborative care model, the Geriatric APRN works to implement evidence-based dementia assessment and treatment practices.The Geriatric APRN is a member of the care team for patients and their caregivers and works closely with the primary coordinator of that team, the Dementia Care Manager (DCM). The APRN will be responsible for assisting with diagnostic evaluations for patients with suspected cognitive impairment and ongoing geriatric dementia care for those with known diagnoses of dementia. They will provide guidance to DCMs and PCPs on disclosure of the diagnosis to the patient and caregiver when required and on optimal dementia care for those with an established diagnosis. The APRN will also be responsible for medication review and consultation, systematically assessing pharmacotherapy and recommending de-prescription of medications associated with adverse effects on cognitive functioning or prescription of beneficial medications when clinically appropriate. (S)he will recommend referrals to specialty care (i.e. Neurology, Psychiatry) within the MGB system when clinically indicated. The APRN will communicate regularly with both the DCM and the supervising dementia expert physician and will participate in weekly case review and weekly team administrative meetings.This role will serve up to three primary care practices. The APRN may meet with patients and caregivers in person or may collaborate with the Memory Care team remotely through telehealth channels, depending on coverage/geographical location. The Geriatric APRN will have experience with assessment and treatment of dementia and working within the parameters of the healthcare system. The ideal APRN will be an outstanding verbal and written communicator, dedicated to high-quality care, and driven to improve the quality of life for adults with cognitive impairment and their caregivers. In addition, to being a member of the Memory Care Team , the APRN will be an integral member of the Division of Palliative Care and Geriatric Medicine, working collaboratively with their inter-professional palliative care colleagues to provide exceptional care to patients/families while promoting effective/efficient clinical operations. The APRN will be highly professional, flexible and collaborative. The APRN is expected to be HIPAA and Joint Commission compliant, follow the Unit''s code of conduct and be guided by the MGH Mission, Credo and Boundaries Attend mandatory trainings related to role and program Will assist DCM with dementia-related assessments for referred adult patients using evidence-based screening tools; e.g. Nine-item Patient Health Questionnaire (PHQ9), Montreal Cognitive Assessment (MoCA), and Eight-item Interview to Differentiate Aging and Dementia (AD8), etc. Provide guidance on assessment of patient and caregiver and contribute interpretation of outcomes to assist PCP in establishing diagnosis, as needed. Guide DCM and/or PCP in conducting difficult conversations regarding dementia diagnosis and care planning. Assist DCM in determining patient disease severity and caregiver level of need; stratify accordingly. Review medication lists and provide input to Memory Care team and PCP, including recommending de-prescribing of harmful medications and prescribing of beneficial medications as necessary. Effectively communicate with members of care team. Participate in weekly case review with DCM and supervising dementia specialist clinician. Also participate in weekly administrative meetings. Recommend referrals of patient to other MGB programs and specialty care as needed, advocate for such services on behalf of patient and caregiver. Promote Memory Care Initiative program within assigned practices, to providers and patients. Collaborates with nursing/hospital education resources to facilitate learning experiences for staff and student/learners. Provide on-call clinical coverage and/or other duties as assigned. Serves as a resource to hospital and clinic staff in area(s) of expertise. Collaborates with nursing and hospital educational resources to facilitate learning experiences for staff and students assigned to the clinic (e.g., Palliative Care Fellows, APRN students) as appropriate. Promotes an environment that is sensitive to the culturally diverse needs of patients and employees. Evaluates current research findings for clinical specialty and determines applicability to clinic practice/evaluates own practice. Identifies pertinent nursing and health delivery problems for investigation, as appropriate Professional Development Engages in self-performance appraisal on a regular basis, identifying areas of strength as well as areas for professional/ practice development. Identifies learning needs and goals on a regular basis and designs a plan to meet them. Maintains necessary clinical skills through continuing education and direct clinical practice. Establishes self as a resource in areas of the hospital/community where expertise may be required. Participates in leadership groups and division/institutional committees, as/if appropriate. Continually evaluates quality and safety performance, identifies improvement opportunities; leads and/or participates in performance improvement activities, if/as appropriate. Collaborates with leadership in identification of current and future clinic/division needs and shares responsibility for meeting these needs. Holds membership in one or more national nursing organization.QualificationsKnowledge of/experience with Alzheimer''s and related dementias, disease management, caregiving, end-of-life care, advanced care planning, psychosocial determinants of health Skill in administering assessments of dementia. Ability to formulate succinct case summaries, document clear care plan goals and actions, and make appropriate referrals; integrates theoretical knowledge base and clinical expertise in decision making. Effective verbal and written communication skills appropriate to the patient populations served. Basic computer proficiency inclusive of ability to access, enters, and interpret computerized data/information, familiarity with EPIC preferred. Ability to function independently and to adapt to a complex, fast-paced medical environment. Dedication to high-quality care to improve quality of life for adults with cognitive impairment and their caregivers. Ability to effectively manage stress due to critical issues related to patient care, changing organizational climate and personnel issues. Demonstrates excellent phone and customer service skills, the ability to multi-task and remain professional in a busy clinical setting. Consistently demonstrates accountability, compassion, and professionalism to internal and external customers. Champions collaborative practice with coworkers and other disciplines and departments.
