THIS POSITION IS ELIGIBLE FOR A $2,500.00 STAY BONUS FOR 1 YEAR OF SERVICEPosition Responsibilities: The Mobile Crisis Case Manager will provide mobile crisis and case management services to patients and their families. The Mobile Crisis program specializes in providing services to patients, families, and other community members who have an urgent mental health need. The mobile crisis team will respond to all approved referrals. During response times, a team consisting of two members will responds. The mobile crisis team will assess the situation and create a safety plan with the individual and/or family. This may include providing multiple services until transitioned to an appropriate level of care. In addition, they will provide support to the Inpatient Unit and answer phone calls as available. Qualifications: Bachelor or Associate degree in Social Work, Psychology, or other mental health related field preferred and one (1) year experience providing direct services to clients in a mental health setting; High school diploma with experience will be considered in lieu of a degree. Day-to-Day Expectations:Implements activities for patients referred to mobile crisis.Creates a safety plan with the patient and, if applicable, includes family and/or natural supports.Consults with on-call therapist and on-call psychiatrist, as appropriate.Provides therapeutic services for patients, including family and natural supports, involved in mobile crisis services including home based services needed to maintain safety and stability.Provides transportation to treatment activities and appointments, when necessary.Why You Should Apply?
