We are currently searching for a full-time Medical Social Worker to support our hospice patients in their homes in the Westmont and surrounding areas. This role will also involve assisting with the bereavement support as well. The Medical Social Worker is responsible for providing patients and families psychosocial care regarding economic, social, and emotional needs. Location 4704 - ProMedica Hospice - Serving Central Chicagoland, Illinois Educational Requirements Masters Degree in Social Work from an accredited institution.May require licensure or certificate as required by the state, and in good standing. (if applicable) Position Requirements Prefer one year experience in a health care setting. 100% travel necessary on a daily basis. Demonstrate problem solving and decision making skills. Demonstrated ability to work will with an interdisciplinary team. Excellent communication skills. Ability to develop and maintain rapport with the client and family.
