Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.JOB SUMMARY The Case Manager (CM) provides case management services in either the community and/or in office setting to patients and their families. The Case Manager assists the patient and care team in planning, connecting, outreaching, advocating, coordinating and monitoring the patient''s needs and progress. The CM assists patients in gaining access to health services, financial assistance, employment, education, social services and natural supports. The Case Manager assesses patient needs, develops and monitors individualized treatment plans and documents services provided in an accurate and timely manner. Embraces the recovery philosophy and promotes a safe recovery environment for clients. ESSENTIAL JOB DUTIES OR RESPONSIBILITIES Coordinates patient care with the CBI Care Team, as well as other CBI facilities and staff. Assists in development, implementation and revision of individual treatment plans; assures that services provided are specified in the treatment plan and monitors progress toward treatment goals. Works in a variety of settings with culturally-diverse individuals and communities with the ability to be culturally sensitive and appropriate. Assists in completing patients'' closures. Share knowledge and experience with patients, as appropriate. Advocate for patients and their communities. Facilitate two-way communication between patients and Care team. Coordination of care with external BH and Medical providers. Outreach and engagement for patients assigned to the site all patients who miss scheduled sessions or appointments. Ensures family members and other natural supports who have been identified by the patient have releases of information on file and are involved in the patient''s ongoing treatment. Participates in case staffing with other care teams as necessary. Writes reports, business correspondence or procedures manuals. Assists patients in identifying community resources to assist in their recovery and encourages patients to attend (12 step meetings, celebrate recovery, smart recovery, support groups and other recovery-oriented groups), to maintain ongoing recovery and build outside support. Demonstrates knowledge of relevant federal and state regulations pertaining to managed behavioral health and insurance products. Reads, analyzes and interprets general business periodicals, professional journals, technical procedures and/or government regulations. Utilizes motivational interviewing strategies to assist patients in increasing their insight and committing to their recovery. Works in coordination with counselors to ensure completion of the patient intake process into Outpatient. Complete monthly treatment reviews with all assigned patients. Complete all error corrections identified by Medical Records personnel within five days of being notified. Ensure all activities are documented in the electronic medical record in accordance with CBI policy and procedures. Complete and document all patient interaction and progress notes within 24 hours, if unable to do so Practice Manager must be contacted immediately. Complete Annual Assessments and ensure a 90% compliance with established standards as monitored by Quality Management Department. Complete Treatment Plans and Demographics every 30 days with a 95% compliance rate and shows evidence in the following areas: Decrease in substance use. Decrease interaction with the legal system. Adherence to medication regimes. Decrease in crisis and inpatient episodes of care. Improvement in physical health conditions. Decrease in high-risk behaviors (sexual, physical health, etc.). Employment and housing access has increased. Increase in symptomatic and functional improvement. Documentation in the clinical file represents services/codes outlined in the Integrated Treatment Plan with a 95% compliance rate. Assists in inpatient, crisis residential or hospital admissions when need has been identified 95% of the time. Participation in discharge planning from hospitalization and inpatient 90% of the time. Other duties as assigned by supervisor and/or manager and/or administrative support. SUPERVISORY RESPONSIBILITIES This position has no supervisory responsibilities. MINIMUM EDUCATION & EXPERIENCE REQUIRED Highschool diploma or GED required. Associates degree or higher in a field related to behavioral health preferred. 1-3 years of experience working with the GMH/SU population. 1-3 years of experience in a behavioral health position providing outreach and engagement activities. 1-3 years of experience in a medical setting preferred. Recovery from Alcohol or Drugs and/or GMH for equal to or greater than 1 consecutive year preferred. Or equivalent combination of education and experience. LICENSES, CERTIFICATIONS, CREDENTIALS, REGISTRATIONS Current AZ Driver''s License Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures Arizona Fingerprint Clearance card Clean 39-month driving record. OTHER SKILLS AND KNOWLEDGE REQUIRED Able to read and comprehend information without difficulty. Able to type 40/wpm. Able to utilize Microsoft systems such as Outlook, Word, and Excel. Able to coherently communicate with others. Able to synthesize complex information and report it to others in a calm and precise manner. Able to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Able to speak clearly and persuasively in positive or negative situations; listens and gets clarification; responds well to questions. Able to work well with others and can be depended upon to follow through with task completion and be available to assist. Able to follow the established chain of command and does not create situations that would call into question the authority of supervisor or impede their ability to manage the facility or program. This is also demonstrated by one''s ability to perform assigned duties in a competent manner without the violation of Community Bridges policies/processes, patient and/or employee rights, or violation of State, Federal, or local laws. Able to perform work neatly and completely. All work is completed within the time allowed for task per policy. Ability to treat community stakeholders (police, fire, probation, businesses, clergy, insurance companies, providers, Health Plan, concerned citizens, etc.) with the utmost respect and assisted to the best of our ability in each case. Applies knowledge and experience to analyze and assist with solving of moderate-to-complex issues involving some ambiguity. Able to develop solutions for improvement of clinical systems. WORK ENVIRONMENT/CONDITIONS AND PHYSICAL REQUIREMENTS Able to pass background check. Able to move around quickly and freely in response to crisis situations. Able to work after hours, weekends and holidays when member requires services. Must be able to move and/or lift a minimum of 150 pounds. Able to drive to multiple locations upon notice or need without restriction. Must be able to complete daily documentation of interaction with member no later than 48 hours after last interaction. Must be able to complete crisis intervention related documentation prior to end of scheduled shift or at the end of the event.
