Bilingual Patient Care Technician (San Diego County)Full-time- RegularMust be Bilingual/Bicultural- Spanish/EnglishField based role with extensive driving in assigned areaAssigned area- San Diego County (Preferably Northwest SD County, must be within a commutable distance for consideration)About HealthCHEC- ComprehensiveHealth Evaluation CenterHealthCHEC- Comprehensive Health Evaluation Center is an innovative member centeredprogram were members of SCAN are invited to complete their Personal Health Assessments.The program has been in operation since 2013. The Personal Health Assessment includes reviewing and completing a detailed medical history, complete review of tests, screenings and preventative care the member has received, review of current medications and a complete physical exam. Upon completion of the Personal Health Assessment, ourprogram develops a recommended care plan, discusses it with the member and sends a report to the member''s Primary Care Physician.A small number of patients requiring on going management of geriatric chronic and advance illness will be followed longitudinally by the HealthCHEC team.Job Summary:The Bilingual Patient Care Technician for San Diego County functions as the Clinic''s initial point of contact, completes front office administrative tasks and provides back office clinical support for the clinician whether during home or clinical visit.Your Key Duties and Responsibilities:Cordially greets and checks in all patients and visitors. Escorts all patients to rooms, as appropriate.Promptly answers all calls; screens and routes calls to appropriate person/department.Responds appropriately to phone inquiries by listening carefully, speaking clearly, and communicating at a level appropriate to meet caller''s needs.Obtains accurate vital signs following protocols approved by Medical DirectorPerforms patient testing as directed by the onsite clinician which may include an EKG, administering a PHQ-9 and other testing as directed.Records patient data in charts in a timely and accurate manner.Maintains examination/treatment rooms, including inventory of supplies and equipment.Prepares and administers immunizations, as directed by the onsite Clinician and as permitted by state law.Recognizes and responds to emergencies following Clinic protocol.Collects and processes lab specimens including blood draws as directed.Performs CLIA-waived tests, as directed by the onsite Clinician and as permitted by state law.Schedules, coordinates, and monitors appointments to support timely and effective care.Maintains reception/lobby area to ensure it is neat, organized, clean and professional.Establishes, organizes, files, and maintains patient medical records, including the timely submission of accurate encounter municates results of the Chronic Care Assessment with patient''s Primary Care Physician.Assists onsite Clinician with coordinating referrals through the Primary Care Physician.Updates and maintains departmental documents, as directed.Identifies, records, and maintains shipping/receiving information for packages and deliveries.Functions as supply coordinator as needed, including order entry, requisition/invoice reconciliation, filing systems and monthly usage reports.Performs general office administrative and support services, typing, copying, faxing, etc. Creates professional looking correspondence, completes purchase requisitions, office supply requests, and other company forms.Opens and closes the office during business hours in accordance with Clinic protocol.Contributes to team effort by accomplishing related results as needed.Actively supports the achievement of SCAN''s Vision and GoalsOn occasion will be required to perform in home visits and care as needed.Other duties as assigned.Your Qualifications:High School/GED requiredCompletion of medical assistant program, required. Certified Medical Assistant (CMA), strongly preferred.Must have experience in palliative, hospice, or home health care.Current Basic Life Support Certification required.Must have 3+ years front and back office experience in a professional medical office environment.Must beBilingual/Bicultural- Spanish/English. Test will be administered to assess language proficiency.Effective, respectful, and courteous communications with internal and external clients required.General knowledge of office equipment.Experience in MS Office.Ability to stoop, kneel and crouch required.
