Senior Coordinator of Quality Improvement Department Quality Management - Hudson Valley Type of Position Full time Search Location(s) Brewster, NY Apply Now (Job Brief The Senior Coordinator of Quality Improvement/ Investigator is responsible for reviewing regularly scheduled quality improvement surveys as part of CP Unlimiteds quality improvement program.Shift Worked:M - F, 9 am to 5 pm with variablesWeekly Hours:37.5FLSA Status:ExemptPosition Summary The Senior Coordinator will review written reports summarizing findings and make recommendations, as appropriate. The Senior Coordinator will provide technical assistance and support to program staff in developing improvement plans to ensure compliance with all applicable regulations in the Residential Program and Day Programs. The Senior Coordinator Advocates for the rights of all people receiving services throughout the Agency by: conducting investigations of situations which may violate a persons rights and/or protections of people receiving services, providing training to Agency wide staff on topics related to the protection of the rights of people receiving our services. Member of Agency standing committees, as assigned. Prepares and presents cases to the Agency''s Incident Review Committee. Position Duties 1. Conducts investigations into situations which may violate the rights and/or protection of people receiving services. These situations include but are not limited to: allegations of abuse, mistreatment, neglect, and violation of civil or human rights. Prepares investigative reports and forwards completed reports to Agency personnel (Human Resources, Legal Counsel, Program Administrators, Incident Review Committee, etc.) and to outside Agencies (OPWDD IMU, CAB, MHLS, Police, Justice Center, etc.)as required. 3. Interfaces with Incident Reporting Management Application (IRMA) to enter approved final investigative reports for Reportable Abuse/Neglect, Significant and Serious Notable Occurrence Incidents as assigned. Ensures that written notifications and follow-up reports related to Reportable allegations of abuse, Significant, Serious Notable Occurrence incidents are forwarded to required outside Agencies (OPWDD IMU, CAB, MHLS, etc.) within mandated time frames as assigned. In consultation with Agency''s Legal Department and Director of Quality Management contacts outside Agencies (i.e. Police Department, District Attorneys Office, Adult Protective Services, Child Protective Services, etc.) as needed. Presents summary of investigative findings to the Incident Review Committee as assigned. Conducts training sessions in abuse identification, prevention, and reporting and on the Agencys incident review process for all staff. Assists QI Specialist as needed for regular quality improvement reviews at both Residential and Day Program sites throughout the year. Completes and/ or reviews Summary of findings and recommendations from quality improvement reviews in a written report and distributes to all appropriate parties. Provides technical assistance and support to agency program staff in developing corrective actions for identified issues, as necessary, to ensure regulatory compliance. Coordinates with the Training Personnel to develop and implement training programs based on findings from quality improvement surveys, investigations and as needed to support programs. Provides technical assistance to agency program staff in addressing areas of concern/deficiencies cited in OPWDD surveys. Assists Director of Quality Improvement in compiling data on a quarterly basis from internal quality improvement and OPWDD surveys. Works collaboratively with management and program staff on the development of strategies, systems, policies/procedures, as appropriate to correct identified issues. Participates in standing committees, meetings, as assigned. Performs all other duties, as assigned. Requirements BA/BS in related human services field. Masters degree preferred. Minimum 5 years experience in developmental disabilities and /or regulatory compliance. Thorough knowledge of City, State and Federal regulations. Excellent verbal and written communication skills. Able to work flexible hours and/or days as necessary.
