Pathway Family Services is an affiliate in Kansas offering additional services to youth and families. Mainstream Nonprofit Solutions is an affiliate supporting the TFI Family of Companies and other Nonprofits.As an Aftercare Specialist, you will promote the well-being of children in the Permanency Services Program, participates in staff meetings and trainings and coordinates with staff from other agencies involved in providing services.• Utilizing best practice standards, supports the Case Management team in managing a caseload of post-permanency placements, with focus on cases at risk of disruption.• Coordinates all permanencies including reintegration, independent living, adoption and custodianship, as defined by grant during the aftercare period.• Coordinates with the Case Management Teams and the Independent Living/Intellectual/Developmentally Delay Specialist as required.• Responsible for providing/coordinating services to children/families, once permanency is achieved in accordance with contract requirements.• Assists the child/family with issues to ensure problems don''t escalate which could cause a disruption.• Provides crisis intervention for the child/family when required. This may also include crisis assessment for any child with an open permanency case.• Coordinates wrap-around services (mental health counseling, drug and alcohol services, educational tutoring, behavior training, etc.) for the child/family to de-escalate and resolve problems.• Coordinates educational requirements for the child.• Coordinates supportive services with local community support agencies for the family/child.• Meets with the child/family as determined by the aftercare plan to support the reunification process and address and resolve problems and issues.• Compiles monthly statistical data, prepares reports and forwards to appropriate agencies.• Maintains a close working relationship with public and private agencies involved in providing services/support.• Maintains the accuracy, content and completeness of child/family case records.• Completes documentation and forwards reports containing descriptive, analytical and evaluative content. Includes a child/family specific monthly report to DCF.• Coordinates with interdepartmental staff, and staff from external public and private agencies on the delivery of services to children and families.• The employee is required to be able to safely operate a motor vehicle and possess a valid license therefore.• Will be required to drive personal or agency vehicle while transporting children or conducting agency business.• Refrains from assuming any duty that is unrelated to and/or interferes with the responsibilities of the position.• Advocates, promotes and practices cultural sensitivity and responsiveness in all day-to-day interactions.• Develops, promotes and practices teamwork in all activities.If you want to make a difference in a child''s life, join our team today!TFI is an Equal Opportunity Employer.To apply, please visit
