RN Field Case Manager - West side City of Chicago

Dynamic RN neededRN who enjoys working with patients and families to teach about medications, self-management of disease process and instructing patient and caregiver how to infuse IV''s, care of central lines, chemotherapy disconnects, foley care, pleurix care, wound care. If you enjoy coordinating care with physicians, other team members, and referral sources this job is for you. You will help patient prevent re-hospitlizations and promote individualized patient care goals. Must be RN licensed in Illinois and willing to cover west side of Chicago from Lake Shore Drive to Austin, North avenue North and Roosevelt Road as the south boundary.
