Case Manager

DescriptionCRi is seeking a dynamic Case Managers for a Part-Time (20 hours with potential for full-time hire) position to work in Montgomery County. This position is eligible for benefits. The Case Manager will serve and support clients with mental health diagnoses and helping them to increase their independence and self-advocacy. The ideal candidate should enjoy working with others in a client''s private home, be engaging and solution-focused, and be comfortable with being in the community for at least 95 percent of the work day.The Case Manager is responsible for all aspects of service delivery to include, but not limited to; completing and updating Assessments and Individual Recovery Plans according to COMAR regulations, connecting clients with community resources (such as medical care, mental health care, food resources, entitlements, housing, etc.), facilitation coordination of care with providers, scheduling and providing transportation, maintaining documentation in the Electronic Health Record, and maximizing the required number of billable units weekly.RequirementsQualified candidate MUST meet the minimum requirements:Bachelor''s degree in a Mental Health field And 1 year Mental Health experience; ORMaster''s degree in field other than Mental Health And 1 year Medical experience; ORBachelor''s degree in field other than Mental Health And 2 years Mental Health experienceMust have valid driver''s license (with no more than 4 points) and a reliable vehicle to use for work.Available to work during business hours.CRi offers company-paid benefits including health insurance options, 403(b) retirement fund and life insurance for benefits-eligible employees.
