We are currently looking for an amazing RN to join our Pediatric Health Center Team! The RN/Diabetes Educator is a professional, Registered Nurse, who will assume clinical responsibility for the care of diabetic patients, along with the Pediatric Endocrinologist and the rest of the diabetes team.Essential FunctionsAssess the patient s past history and current condition, establish and monitor the patient/family goals and outcomes.Assess and implement patient/family education and home care needs.Use an understanding of the principles of growth and development to assess patients age-specific needs and age-specific treatment. Specifically responsible for review and management of patient blood sugar levels, including the application of the Continuous Glucose Monitoring System and insulin pumps. Plan and conduct patient education classes as form patients of lab results, patient education tests such as training patients to administer medications at home, and administration of injectable medications in the office as required. Complete diabetes education sessions and follow up in a timely manner as per policy and procedure. Seek to provide the highest quality service in an accurate, thorough, and timely manner. Demonstrate practical time management skills.Assist with non-diabetes pediatric endocrinology patient care as appropriate. CDCES Responsibilities Educate newly diagnosed patients with diabetes Inpatient and Outpatient Education. Participate in newly diagnosed / sick day patient phone education and support.Additional education as needed including insulin pump topics, sensor education, and insertion municate with School Nurses regarding care plans and school-related issues.Suggest Insulin Dose Changes Between Physician Visits after reviewing uploaded blood sugar information. Perform diabetes education portion of physician visits.Assist with Certificate of Medical Necessity paperwork for diabetes supplies, insulin pumps, and continuous glucose monitors. Qualifications: Current New Jersey licensure as a Professional Registered Nurse in good standing is required. A Bachelor''s Degree is strongly preferred.Graduate of an accredited school of nursing is required.One-year prior experience as a registered nurse in community health or adult medicine is strongly preferred. ''''Intravenous Therapy'''' / Venipuncture certification is strongly preferred. Current Certified Diabetes Care and Education Specialist (CDCES) certification preferred but will train the right candidate.Knowledge, Skills, Abilities: Basic keyboarding skillsMust be able to communicate effectively by speaking clearly and distinctly in English.Cerner / EPIC EMR experience is desirable.Maintain required BLS Health Saver / Health Care Provider course completion status or acquisition within 90 days of hire. Appropriated Advanced Life (PALS or ACLS) Certification preferred.
