SummarySupports the individualized service plan and dementia programming for residents by administering medications, observing treatments, maintaining current health profiles to ensure quality of care and assisting residents with personal hygiene and activities of daily living while promoting independence as necessary or as directed.Duties and ResponsibilitiesCommunicates with care attendants to ensure effective care. Advises and coaches as necessary.Participates in residents plan of care by implementing, evaluating and monitoring service plans to enhance quality of life, residents rights, privacy and municates with families regarding resident needs during municates changing resident needs and concerns to Social Worker to ensure coordination of care.Promotes safety in care and surroundings.Executes physician orders to coordinate residents prescribed care.Responsible for accurate reporting and documentation to ensure good communication and thereby provide for effective continuity of care.Coordinates resident appointments to expedite medical care.Evaluates resident healthcare including identification of needs, communication with physician and coordination of physician orders.Provides supervision to residents and/or assistance with toileting, eating, various personal care services such as bathing, dressing and grooming to effectively meet individual residents needs as necessary.Conducts planned and spontaneous activities with individual residents or small groups to promote the highest possible level of resident functioning, while promoting self - esteem.Has knowledge of and implements emergency measures to ensure resident safety and comfort in the event of accident, fire and disaster.Responsible for dispensing medications, monitoring for changes and maintaining current and accurate medication records.Attends annual Woodside Place retreat.Participates in dementia specific training 6 hours annually and annual training as required by DPW regulations.Supervisory ResponsibilitiesProvides oversight and crisis management for 3-6 Personal Care Aides.Education and/or ExperienceCurrent LPN license. 2 years dementia care experience preferred but not required.Certificates, Licenses, RegistrationsValid LPN License in good standingThanks for your interest in Presbyterian SeniorCare Network!Location: Washington Campus • Personal CareSchedule: Full Time, Evening Shift, Premium Evening Program New rates $29.00hr.
