The incumbent is assigned to perform a clearly defined clinical function permitted by state communicable disease guidelines. Incumbent will be functioning as part of a multidisciplinary team under the direct supervision of the Assistant Community Health Nursing Director. Initiates and maintains patients'' records, makes nursing assessments of patient needs. Enters HMS codes, current procedure terminology and procedural codes into the Health Management System. Maintains an organized system for following up with clients. Assists in the follow-up of communicable and sexually transmitted diseases, abnormal pap tests and refers to appropriate agency as needed. Counsels and supports clients/families that have experienced HIV or any devastating disease. Acts as liaison between client and community agencies. Registered nurses shall be responsible for the following activities as consistent with the nurse practice act: history taking, physical assessment, care referral, counseling/education, health promotion and risk reduction counseling, and referring patients that require disease intervention counseling to the DIS. The RN may also perform sample collection for lab testing. All complex and multi-problem cases shall be referred to licensed clinicians for collaborative case management. Assesses plans, develops, implements, and evaluates treatment programs that promote health and well-being; treatment plans for patients with a known long-term condition; and treatment plans for patients at risk of developing a long-term condition as appropriate. Works with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs. Assesses complex patient needs/situations and consults with Physician and/or Advance Registered Nurse Practitioner as needed. Assists in the planning process and orientation of new staff and provides clinical experience for university nursing students. Represents local STD unit on various committees within the agency and the community. Coordinates STD clinic activities in the absence of the supervisor. Relieves nurse in another STD clinic when needed. Conducts peer reviews of medical records. Monitors inventory of pharmaceutical supplies and works with the Asst. CHN Director to ensure supplies and medications have been ordered. Attends staff meetings, trainings, in-service programs, conferences, and workshops while maintaining educational requirements for nursing licensures. Performs other related duties as required or directed. Required Knowledge, Skills, and Abilities: Experience maintaining accurate, detail reports and patient records. Ability to perform comprehensive patient assessment. Knowledge of specimen collection techniques. Knowledge of methods of administering medications to patients and monitor patient reactions. Knowledge of methods of health educations, promotion and counseling. Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction. Ability to maintain strict confidentiality. Qualifications: Minimum - Licensure as a Registered Professional Nurse in accordance with Florida Statute 464 or eligible to practice nursing in accordance with Florida Administrative Code 210-8.22 or 8.27 Preferred Preferred to be bilingual in English/Spanish or English/Creole.
