The incumbent has oversight of the HIV/AIDS program and is responsible to perform functions requiring independent judgment while maintaining positive working relationships with all agency divisions, community agencies and general public. This individual will be responsible for all supervisory duties including, but not limited to, hiring and selection, developing performance plans, conducting performance appraisals, planning and directing work, approving leave, and taking disciplinary action as appropriate. This position is responsible for providing leadership and direction to HIV/AIDS prevention, education, and patient care programs in Area 12, (comprising of Volusia and Flagler Counties). Serves as the Area 12 HIV/AIDS Program Coordinator (HAPC). Acts as a community liaison for Area 12 to expand community footprint bridge the gap between the CHDs, CBOs, providers and other organizations and the HIV/AIDS program office. Scope of responsibility includes community engagement and mobilization, public information and media relations related to the HIV program and strategic plans programmatic planning, carrying out quality assurance standards, monitoring, supervision of surveillance and program support staff, and program evaluation. This position reports to the County Health Department Administrator and will be expected to support integration efforts to enhance disease control programs (EPI/STD/TB/HIV/EH) capabilities. Program Development/Supervision and Management Assists with monitoring and evaluating the performance of the HIV/AIDS program including clinical, case management, and prevention/education activities using quality assurance, prevention, counseling & testing and other programmatic and fiscal guidelines. Develops, updates, and implements quality assurance guidelines for monitoring the quality of HIV/AIDS services provided in the two-county area. Assures that the statewide and Area 12 goals and objectives are achieved and maintained . This position will supervise the Surveillance program, Area 12 Administrative Support Staff Supervisory duties include, but are not limited to: hiring and selection, developing detailed performance standards and position descriptions, conducting performance evaluations, planning and directing employees work, approving leave and taking disciplinary action as appropriate. Plans and distributes workloads; monitors and evaluates workflow. Assists in identifying problems, seeks solutions, and implements change. Maintains an effective work unit. Keeps supervisor informed on all issues. Maintains effective inter- and intra-departmental communication. Communicates and clearly articulates DOH expectations regarding professional conduct, time and attendance policies. Develops and provides timely orientation for new employees. Coaches, trains, mentors and empowers staff to reach their full potential by working with them on a daily basis. Evaluates the work of all staff on a regular basis. Develops short and long-term program plans, goals and training tools for staff. Oversees HIV Surveillance and administrative assistant to support HIV prevention and patient care programs. Respond appropriate and in a timely manner to requests for reports, publication, proposals and budget information. Coordinate with HIV/AIDS staff to identity serve gaps and develop a plan for addressing. Provides assistance to ensure county health department HIV/AIDS services are available. Participates in QI/QA reviews, follow up on recommendations for improvement. Ensures local procedures are developed, as needed or required, to implement HIV/AIDS statutes, rules and operating procedures. This position will be expected to cross train to provide back-up support and duties for the HIV program, and support integration efforts to enhance disease control programs capabilities and ensure coverage. Keeps Supervisor and other stakeholders routinely informed of changes noted or issues of concern. Ensures local and statewide data is shared, assists community in obtaining appropriate data. Facilitates coordination and communication between surveillance staff and other internal programs and outside relevant entities. Maintains relationship and regular communication with Area 12 HIV/AIDS points of contact and their direct supervisor/administrators. Researches and secures additional funding to meet identified needs in Area 12 with prior approval from supervisor and finance director. Participates in the monitoring of HIV/AIDS programs funding and tracks spending patterns assuring compliance with federal and state grant procedures. Consults with federal, state, and local officials as directed and required. Community Liaison/Mobilization Plans, develops, and coordinates an Area 12 HIV/AIDS education program for professionals, the community, and for providers. Educates the community on the bureau goals, objective and available programs and services (i.e., thought presentations, materials, media, etc.) Ensures accurate and current HIV/AIDS information is available for providers, planning bodies, community and media. Coordinates with county health departments and community-based organizations to provide a network of educational opportunities available in Area 12. Represents the Department at regional and statewide meetings related to HIV/AIDS and other related program issues. Assist the county health departments and community partners to develop more responsive and comprehensive systems of HIV/AIDS care. Provides leadership to community-based organizations and consortia as well as coordinating with the state disease interventions services unit and with the local Volusia County Prevention Planning Group and state HIV Community Planning Partnership. Monitor, coordinate, provide leadership, and participate in the activities of the Ryan White Part B Consortium Care Network. Acts as a liaison between the State, Area 12 CHDs CBOs, Providers, Political, Community Members and Consortium. Provides technical assistance and training to the and to HIV/AIDS Services Contract Providers involving all areas of HIV/AIDS assessment, planning, care and treatment. Participates in and meetings. Assists the with needs assessment and strategic planning activities. Provides technical assistance on HIV/AIDS activities. Participates in planning efforts to reduce new HIV infections; increase access to care and improving health outcomes for people living with HIV; reduce HIV-related health disparities. Collaborates with community partners and providers to achieve the goals set forth in local, state and federal plans across program areas. Carry out and/or review, conduct analyses, including identification of trends in health determinants, health outcomes and ecologic and individual risk factors related to HIV/AIDS of specific communities. Develop community reports that showcase these health determinants, health outcomes and risk factors in our community. Present reports to medical community, media, public health staff and the general public; plan in collaboration with PIO optimal methods and avenues for disseminating information. Advise Director, senior management and community partners on HIV/AIDS issues and community health findings; represent the Department on a community level; consults with stakeholders to guide policy making. Actively participates on the agency SLT, PMC and Healthy Volusia. Attends program, agency, and community meetings as required and needed. Assists Accreditation Coordinator in ensuring compliance with PHAB Accreditation Standards and Measures for agency overall. Maintains documentation and provides regular updates to supervisor and Accreditation Coordinator. Assists with other areas of performance management and quality improvement for the agency as needed, including assuring compliance with agency strategic plan and divisional goals. Identifies problems and proposes and monitors corrective actions related to assigned performance measures to ensure achievement of goals. Assists with grant writing and proposal development and review. Takes an active role in community health improvement planning and developing community health profile reports.
