OverviewAbout St. Christopher''s Hospital for ChildrenSt. Christophers Hospital for Children, a partnership between Tower Health and Drexel University, is a 188-bed facility that provides exceptional care to children throughout the Greater Philadelphia area and surrounding counties.St. Christopher''s has more than 220 pediatric experts on staff, combining top-notch pediatric care with a wide array of pediatric specialties including Cardiology, Ear, Nose and Throat, Gastroenterology, Oncology, and Orthopedics. It houses the only Verified Pediatric Burn Center between New York City and Baltimore and is one of only three Level I Pediatric Trauma Centers in Pennsylvania. St. Christophers is a Magnet designated hospital and was recognized as a Womens Choice Award Best Childrens addition to its main location in Philadelphia, the hospital has a growing network of primary and specialty care locations throughout the Philadelphia suburbs and New Jersey so your child''s health conditions can be treated close to home.We also offer a robust Nurse Residency Program which all new graduate professional nurses will be enrolled into upon hire.ResponsibilitiesThe Critical Care Transport Nurse (CCTN) provides patient care in a mobile environment to high risk patients and their families being transported to/from St Christophers Hospital for Children. The CCTN acts as a resource hospital wide for patient care standards and pre-code recognition. As a recognized resource, the CCTN assists in establishing and maintaining a high quality standard of care for the neonatal and pediatric transport population. Within the call center, the CCTN acts as a resource to community and hospital based physicians for pediatric consultations, transfer recommendations, and a smooth transition to and from St Christophers Hospital for Children.Essential Functions: Demonstrates effective behaviors as outlined in the organizational core competencies.ClinicalTransport: Functions as a team leader when a physician is not present on the transport. Assesses patient and provides interventions according to PALS, ACLS guidelines, and St Christopher Hospital for Children pathways in an emergent situation, or after consulting with medical command physician. After consultation with Medical Command, exercises independent judgment utilizing critical thinking skills in situations when sudden clinical changes occur. Understands and implements necessary interventions and their expected outcomes. Effectively communicates with parents and families throughout the transport process. Understands and satisfies the needs of the neonatal, infant, toddler, pre-school, school-age, adolescent and/or adult patient in regards to their growth and development process. Provides a safe environment while maximizing clinical expertise during interfacility transports. Documents care during transport including all demographics, vital signs assessments, procedures, medications administered, pertinent history and physical exam, provisional diagnosis, plan and orders for care along with response to any interventions provided. Collaborates with other team members in the planning and provision of optimal patient care during the transport process including patient placement. Contributes to the identification of a potential diagnosis, interventions, and expected outcomes specific to the defined patient population. Possesses analytical and critical thinking skills in interpreting patient labs and overall patient assessment. CXR and any other diagnostic studies when available.Department Call Center Operations The CCTN facilitates the transport process by obtaining pertinent patient history and clinical status, communicating information with medical command, obtaining acceptance from the receiving attending physician, notifies nursing supervisor, arranges mode of transport, and dispatches the team within the individuals scope of practice. Facilitates communication for physician referral requests between community based physicians and specialty services within the hospital. Assists in the Direct Admission process. Departmental Assists in the development, implementation and evaluation of St. Christophers Transport departmental operations, policies, procedures, products, programs relating to the transport environment and patient population. Acts as a team member by identifying personal, departmental and hospital goals. Collaborates in a positive manner towards the success of the goals by offering input, ideas and solutions. Assists in the development, implementation, and evaluation of Quality programs for the Transport Team. Maintains department responsibilities in Transport by attending educational offerings, staff meetings, departmental and hospital interdisciplinary meetings, case reviews, and recurrency training.Education Assists in the development and validation of patient care standards for the transport patient population. Develops educational material for staff, referring staff, parents, patients, and pre-hospital personnel. Assists in the identification of patient care provider educational needs. (SCHC staff, pre-hospital personnel, referring facilities staff). Obtains and maintains required certifications and licensure. Provides for and maintains professional growth of self and others.WORKING CONDITIONS:Provides services in a mobile transport environment where there is potential for exposure to blood and other high risk body fluids, communicable disease and hazardous materials, chemicals, waste, temperature extremes, crowded work space, loud noises, vibrations, and vehicular breakdowns and crashes.There is potential for personal injury from equipment or instruments and vehicular crashes.LIMPSC21Qualifications Graduate of an accredited School of Nursing Acquires and maintains all Joint Commission requirements, BLS, ACLS, and PALS certification. PHRN (prehospital registered nurse) certification or obtain within 1 year of hire. Demonstrates analytical skills necessary to resolve complex patient problems in a mobile environment, assists in developing standards of care and procedures, and evaluate the quality of nursing care. Demonstrates ability to work independently and utilize time management, prioritization and multi-tasking skills. Demonstrates ability to assemble complex equipment and perform tasks which require fine motor skills.Demonstrates ability to cope with physically and mentally stressful situations. Basic personal computer skills required, advance skills preferred. Maintains and demonstrates the ability to rotate shifts and/or work flexible hours to meet unit staffing and patient care needs. Must pass pre-placement physical exam and meet health requirements for flying within the physical capacity evaluation. Transport team must meet and maintain weight equal to or less than 230 pounds. Weight requirement is subject to annual review with one year advance notice to change. Must be able to walk, stand for long periods, over 80% of the assigned work Must be able to squat occasionally and sit frequently. In the Ambulance/ Rotor Wing, must be able to lift/carry 50 pounds, push/pull up to 300 pounds, reach overhead moderately, climb moderately, squat/bend/kneel occasionally, sit/stand moderately, walk/move about frequently, tolerate tight quarters, small spaces and sit in a confined space. Possesses three years pediatric critical care nursing experience or transport experience, in order to develop a comprehensive understanding of patient care within a defined patient population, and gain knowledge and skill in pediatric nursing procedures and standards.
