More information is available at purpose of this position is to assist clinical staff in providing quality patient care by advising physicians and other clinical staff on the selection, dose, interactions and side effects of medications.Review all patient charts for correct medication therapy and documentation; including patient drug allergies, drug/drug interactions with current medications, appropriate medication choice, route of administration and doseConduct weekly on-site visits to each facilityConduct monthly unit review of departmentAssist Hospital Administrator to ensure sufficient medication is in stock; including working with the pharmaceutical wholesaler to find alternatives, if necessaryReview and reconcile controlled substances logs and conduct an annual controlled substance inventoryComply with all regulatory agencys rules and regulations as applicable to pharmacyMaintain appropriate facility licenses State Board of Pharmacy, DEA and state controlled substance lincense, if applicableAssist clinical staff with drug related informationProvide on-call 24 hour service to answer questions or assist clinical staff if neededCounsel patients on medication therapy, if neededPlan, monitor and evaluate drug programs and regimensAttend staff meetings or other company sponsored or mandated meetings as requiredTravel to all facility locations as requiredPerform additional duties as assignedBachelors Degree in Pharmacy from an accredited school of pharmacy, requiredLicense in good standing to practice pharmacy issued by the State Board of Pharmacy, requiredPosition requires fluency in English; written and oral communicationID: 2020-12229 Shift Type: Days External Company URL: Company Name (Integration): Dignity Health - St. Rose Dominican Neighborhood Hospital - North Las Vegas Street: 1550 W.
