Acts as a content expert in this area of practice, setting evidence based standards and guidelines for patient care. Is a resource person for the healthcare team, communicating with the patient, family, and interdisciplinary team both verbally and in the EMR.Minimum Education: Masters of Science in Nursing. Valid and Current NYS Registered Nurse Practitioner license. Board certified as Palliative Care Nurse Practitioner ACHPN or an Adult or Family Nurse Practitioner by the AANP or ANCC.Minimum Experience: 35 minimum years of direct patient care experience required as RN in med/surg, oncology, or intensive care. 3-5 years experience in Palliative Care / hospice training.Required Certification/Registration: ACLS/BLS certification necessary. Prescriptive authority and DEA number (preferred)Physical Requirements: Prolonged, extensive, or considerable standing/ walking. Lifts, positions, pushes and / or transfers patients. Considerable reaching, stooping, bending, kneeling, crouching.Working Conditions: Busy and fast paced. Frequent contact with patients, family and other employees.
