The Clinical Pharmacist (Pharmacist 3) will provide pharmaceutical care to clinic patients based on departmental and clinic-based standards of practice; coordinate student/resident clinical activities; participate in Staff Development program including Journal Club, group teaching and student presentations; participate in Ambulatory Pharmacy QI, DUE, outcomes and workgroup(s); participate in development and implementation of clinic-specific patient care programs; and perform and supervise distributive functions in the clinic-based or outpatient pharmacy.This position will help cover pharmacist clinical services in the following ambulatory clinics: Medical Specialties and Cardiology.RESPONSIBILITIES: Optimize pharmacotherapy:• Collaborate with providers to individualize drug therapies promoting safe, effective and economical patient care.• Assume responsibility for monitoring and modifying the drug regimen when referred, to include the ordering and interpretation of laboratory testing.• Review clinic prescribing patterns with clinic staff to design targeted education to promote consistency with UWP guidelines (or other nationally recognized guidelines when needed)• Provide refill authorization in conjunction with assessment of adherence, disease states, drug interactions, efficacy, and safety.• Act as a drug information resource to providers, nursing staff and patients.• Identify patients with indications for drug therapy who are under-treated and optimize drug therapy.Pharmacotherapy Education:Patient:• Identify barriers to adherence to medication regimen, incorporating patient characteristics i.e. language, culture/religious practices, physical, cognitive, emotional, pain/discomfort, socioeconomic status.• Utilize state of the art tools in educating the patient on self-management of disease and drug therapy.• Participate in existing or develop interdisciplinary group education classes that offer support and/or provide an efficient method of information exchange.Clinic Staff:• Provide regularly scheduled pharmacotherapy presentations and in-services• Provide drug information supported by evidence-based practicePharmacy Department:• Participate/present at scheduled pharmacy clinical conferences, journal clubs, workgroup meetings and clinic-based conferences.• Provide continuing education to other pharmacy providers in area of expertise.• Collaborate with Pharmacy Residents on Pharmacy and TherapeuticsPharmacy Students and Residents:• Provide teaching to Pharm.D. students, residents, and staff pharmacists on pharmaceutical care, chronic disease management, distribution and drug information.• Develop and maintain objectives, monitor progress, and assist with projects of pharmacy students and residents. Attend teachings sessions such as journal clubs, group education sessions and student/resident presentations.• Obtain clinical faculty appointment and maintain connection with School of Pharmacy for didactic and experiential training opportunities.Facilitate Observance of HEDIS guidelines:• Focus on pharmacotherapy as one aspect of achieving focused outcomes for various disease states. This includes both consulting with the provider and individual patient visits with the pharmacist to promote the safe and appropriate use of medications.• Utilize evidence based clinical guidelines with UWP guidelines.• Participate in clinic TQM (CQI) teams, development of clinic-specific DUE, and -institutional QI activities.Pharmaceutical Dispensing activities:• Prepare and dispense medications in clinic-based or outpatient pharmacy• Conduct a thorough patient profile review to assess the appropriateness of the prescribed pharmaceutical including monitoring individual patient profiles for drug duplications, interactions, allergies, appropriate dose, strength, and route of administration at the time each order is received. Document relevant information in the patient medical record.• Evaluate all prescribers'' orders and verify correctness of entries in computerized patient profiles• Interview patients, assess barriers to medication adherence and provide patient-centered education regarding their medications.• Counsel patients on medications dispensed reviewing indication, proper administration, side effects, duplications, etc.• Report and document adverse drug reactions.• Comply with all pharmacy laws and regulations• Provide continuity for daily operations and patient interactions• Direct the work of technicians, interns, pharmacy students, and residents• Maintain accurate records of controlled substances dispensed.Research:• Participate in collaborative research with the Schools of Pharmacy, Medicine, Nursing, and Public Health• Participate in collaborative research with members of your clinical team.Other:• Maintain a strong clinical foundation based on primary literature and critical assessment of evidence based practice issues.• Publish articles in professional journals and make presentations at the local, state, and national level that will be useful to other practitioners.• Maintain effective communications with the other clinicians and administrative staff of the Department of Pharmacy and other departments in the Hospital.• Promote the image of service and good will to patients, public, and medical center staff.• Participate in professional pharmacy organizations.REQUIREMENTS:Two years of experience in pharmacy practice.
