Job Description

Clinician I has the primary responsibility of monitoring the individuals on the Developmental Disability (DD) Waiver Waiting List. Will monitor and update client information on the WaMS website. Will assess and update level of functioning and waiting list eligibility. Will work with individuals and families to assist with maintaining their records in the WaMS system and updating of DBHDS required forms. Will assist individuals and families with accessing the WaMS system and identifying resources as needed.Typical Tasks Include: Complete annual VIDES assessments, Priority Needs, and Critical Needs assessments of individuals on the waiting list Document all contacts in the EMR system Conduct provider search and develop and maintain a provider specialty list Complete SARFs for all individuals who don''t receive active case management services Coordinate the submission of National Core Indicators Surveys Submit and monitor requests for initial SIS assessments and re-assessments Conduct home visits Identify and utilize community resources and provide assistance to obtain resources Inform and assist individuals/families on the waiting list with applying for IFSP funding Communicate effectively with individuals Provide case management services to assist the program when assigned Performs other related duties as assigned. VOCATIONAL/EDUCATIONAL REQUIREMENT: Requires any combination of education and experience equivalent to a bachelor''s degree in social work, psychology, closely related field, or another degree as approved by the Virginia Department of Health Professions. EXPERIENCE REQUIREMENT: In addition to satisfying the vocational/education standards, this position requires a minimum of one year of related, full-time equivalent experience. Qualified Mental Health Professional Eligible (QMHP-E) may be considered in lieu of experience. SPECIAL CERTIFICATIONS AND LICENSES: Depending on the assignment, may require a valid driver''s license and a driving record that is in compliance with City Driving Standards. Candidates must be Qualified Intellectual Disability Professional (QIDP) OR Qualified Development Disability Professional (QDDP) certified upon hire. Depending on the assignment, may require a Qualified Mental Health Professional (QMHP - Adult or Child). CPR, First Aid, and/or Medication Administration certification within 90 days of hire; and successful completion of Intellectual Disabilities Orientation Workbook and/or Case Management Modules within 30 days of hire. SPECIAL REQUIREMENT: Employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies.
