Are you enthusiastic about working in community outreach? Are you seeking a career working with a supportive team and wonderful patients where you can make a positive impact on the health of others? EBNHC is seeking a Community Health Worker (CHW). In this role, you are a member of the Medical Home who collaborates with other staff in the Family Medicine Asthma/COPD Program to positively improve the health of Boston area and Winthrop residents living with asthma and COPD. You will hold and attend community events in Winthrop and work closely with the Public Health Nurse. You will deliver asthma/COPD education and environmental/home interventions to improve asthma/COPD symptom control and long- term asthma/COPD management in person and over the phone. In the home, the CHW will provide environmental asthma kits, develop a plan and reinforce medication adherence. You will support the delivery of services to patients/staff of the Family Medicine Asthma/COPD Program; develop efficient workflows by assisting in scheduling medical appointments, and facilitate community resource access. The CHW will work with the Family Medicine Asthma/COPD Program Nurse and Nurse Practitioner. Full time position, Monday-Friday, 8:00 AM -5:00 PM. Bilingual (English/Spanish) required.Home Visits:Outreach to eligible Asthma/COPD patients identified for possible home visits to schedule initial visit.Schedules subsequent home visits per protocol including intake visit, home walk-through and follow-up visits.Provides items in asthma/COPD toolkit per home visit protocol with supporting education and develops and environmental action plan.Uses motivational interviewing techniques to support change and self-efficacy.Refers to appropriate inspectional services departments as needed or outside community resources.Assist the families in the home with the completion of forms for Immigration, Legal/Housing Services, and connection to community resources.Accompany and attend:Specialist appointments, home inspections, etcRemind patients of appointmentsAsthma Education:Organize and replenish asthma related teaching materials throughout the department.Provides asthma/COPD related education to patients in clinical setting per staff need Experience:Understanding of language, culture, and socioeconomic circumstances and desire to work with diverse population is a must.
