DescriptionProvidence is calling a Community Health Worker III (Full-time/Day-shift) to Providence California Regional Services in San Pedro, CA.Apply Today!Applicants that meet qualifications will receive an email with some additional questions from our Modern Hire system.Please upload a current resume reflecting all relevant experience.We are seeking a Community Health Worker III who will be responsible for helping community residents and Medical Center patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW-III provides primary job duties within at least one of the following tracks:a) Health Education: CHWs in this track will assist with recruitment and administrative tasks for health education courses, such as but not limited to Mental Health First Aid, Creating Healthier Attitudes Today, physical activity, or Groceryships. In addition they will deliver these modules these health education topics independently and identify new locations where the programs can be delivered.b) Outreach and Enrollment: CHWs in this track will provide outreach in community settings and provide initial screening for eligibility of Covered CA, Medi-Cal, and CalFresh (Food Stamps). In addition they will provide one-on-one health insurance and/or CalFresh enrollment application assistance. In addition they will also troubleshoot complex cases independently and provide small group presentations on how to use insurance and basic health care rights.c) Navigation/Linkage to Resources: CHWs in this track will manage complex patient subpopulations, including but not limited to people with co-morbidities, recently discharged patients from the Medical Centers or community residents struggling with substance abuse. When necessary, the CHW-III will be paired with a Social Worker to manage these individuals. As part of this they will provide individualized assessments of risk factors that impact an individual''s well-being. They will be responsible for doing the assessments in community and Medical Center Emergency Department settings, then referring them to the appropriate programs and resources in the community, and providing follow-up to make sure the needs are this position you will:General Outreach Duties:Actively seek opportunities for outreach with a focus on hard to reach populations and our identified high-need communities.Conduct presentations about available services to local community based organizations.Maintain and strengthen contact with key stakeholders related to program updates.Work with other caregivers within the Community Health Department (e.g. Outreach Specialist, other Supervisor, etc.) to coordinate outreach events and direct service opportunities; communicate outreach events to Outreach Specialist to ensure Outlook calendar is updated with event information (time, type, location, assignments).While performing outreach activities, recognizes possible community resources and brings back information to supervisor.Maintains knowledge of Providence Community Health programs and provides community with appropriate linkages as requested.Acts as representative for Providence Health and Services at professional and network meetings, as needed.Attend and participate in regular staff and department-wide meetings.Attended trainings, as required by core leader to stay up-to-date on programs.Health Education Instruction Duties (for CHWs in the Health Education track):Identify community members/patients who would benefit from the health education courses offered by the Community Health Department.Refer identified community members/patients to caregivers that run the respective program.Assist caregivers with administrative tasks associated with the implementation of the health education courses.Identify materials appropriate for the target audience and submit materials to Supervisor for approval before being distributed in the dependently deliver accurate and professional health education classes for community members and community organizations, as requested by core leader. Classes are activity-based, demonstrate a skills progression and provide opportunities to practice skills appropriate to the intended target audience.Establish and maintain trusting relationships with the community.Identify new locations where courses can be provided.Revise, prepare or identify materials appropriate for the target audience.Enrollment Duties (for CHWs in outreach and enrollment track):Provide outreach in the community to increase awareness of Covered CA, Medi-Cal, and CalFresh.Administer screening tool to identity and refer community members/patients who are eligible for Covered CA, Medi-Cal and/or CalFresh.Provide on-site and home-based assistance of enrollment and renewal applications to families from partner organizations (clinics, schools, churches, businesses, etc.).Builds relationships with patients/clients by asking appropriate questions in a compassionate manner and uses good listening skills.Follows up and assists families who are having difficulty with enrollment process.Conducts proactive community based follow up with families to make sure they stay enrolled and renew their application on time.Administers client satisfaction survey to clients assisted.Fully understands and is able to explain insurance and CalFresh programs (eligibility, benefits, application process, requirements, premiums, co-pays) and other community health services.Maintains professional and technical knowledge by attending educational workshops or reviewing professional publications to stay informed of eligibility and benefit changes to insurance programs.Works closely with Community Health enrollment and emergency room caregivers to assure a seamless flow for qualified uninsured adults to gain access to Medi-Cal or Covered CA.Troubleshoot complex cases independently.Provide small group presentations on how to use insurance and basic health care rights.Navigation/Linkage to Resources Duties (for CHWs in the navigation/linkage track):Manages complex patient subpopulations, including but not limited to people with co-morbidities, recently discharged patients from the Medical Centers or community residents struggling with substance abuse.Works collaboratively and professionally with a Social Worker when necessary to manage complex patient subpopulations.Provides on-site screenings of risk-factors that impact an individuals well-being in community and Medical Center settings.Refers clients/patients to the appropriate programs in the community that meet the needs identified in the screenings.Builds relationships with clients/patients by asking appropriate questions in a compassionate manner and uses good listening Medical Center settings enroll patients into Hospital Presumptive Eligibility (HPE) and provide enrollment assistance or refer patients to CHIP for patients that need to be enrolled in Covered CA or CalFresh.Follows up and assists families who are having difficulty accessing resources to which they were referred.Administers client satisfaction survey to clients assisted.Fully understands and is able to explain the issues for which they are screening and the resources available to meet those needs.Works closely with Community Health and Medical Center caregivers to ensure a seamless flow for the patients.Documentation/Information Management:Document all outreach events and direct services including maintenance of logs and sign-in sheets.Implement and monitor collection of programs evaluation data (e.g. surveys).Ensure that program statistics are entered into the appropriate database (e.g. Carescope or other Community Health database) per department pile and review monthly reports related to expenses and service units.Submit monthly reports to Supervisor and/or Department Data Specialist.Development:Develop, maintain and demonstrate up-to-date knowledge on community health issues by attending seminars, trainings, or conferences as required by the Department, reading materials or other methods.Develop, maintain, and demonstrates up-to-date knowledge of hospital and community resources.Stay up to date with current literature to ensure curriculum is based on best practices.Participate in staff development activities.Required qualifications for this position includes:Education to meet certification, license or registration requirementCA drivers licenseFor CHWs providing outreach and enrollment, become a Certified Enrollment Counselor (CEC) and be certified to enroll into Cal Fresh within 3 months of hire1 year of Experience in a combination Community Health Worker I and II position, or 2 years related community outreach, enrollment, education or other community health administrative experienceAbility to work collaboratively with others and to participate fully in a team processStrong public speaking skillsExcellent interpersonal skillsAbility to complete necessary tasks and meet deadlinesAbility to follow directions, be a
