Registered Dietician NutritionistThe Registered Dietitian Nutritionist provides a wide variety of out- patient nutrition services including; nutrition assessment, individualized patient care plan development, individual medical nutrition therapy and facilitates educational classes.Essential Functions:Patient Centered Nutrition Care Process Provides nutrition education services including; nutrition assessment, nutrition diagnosis, treatment, monitoring and evaluation of patient treatment. Treatment may include generalized nutrition education and counseling and/or medical nutrition therapy on a specific disease or lab abnormality. Treatment may also include referrals to other One Community Health services and referrals to local food banks, and/or federal food assistance programs such as WIC and SNAP. Document all patient contacts, education, recommendations and referrals within the electronic medical record. Recommends nutrition education products and materials as needed. Conduct outreach by making phone calls to the patients that are assigned to schedule appointments with the RDNs. Nutrition Counseling and Medical Nutrition TherapyWorks as part of the medical team assessing patients; reviewing lab reports and medical records, confers with primary care providers and ancillary providers. Uses evidence based treatment protocols to determine what diet prescription and education process is most appropriate. Provides disease prevention education and treatment, and culturally sensitive dietary counseling within an individualized care plan. Nutrition Related Prescription DocumentationHelp provide necessary nutrition related documentation to primary care providers as needed for insurance coverage and dispensing of nutrition supplements.Documentation and Billing in Electronic Medical RecordAbility to utilize electronic health record and document all nutrition related encounters in clearly and concisely summarizing patient encounter. Uses appropriate billing codes and system to accurately bill for problems addressed and time spent with each patient.Nutrition Promotion and EducationPromotes healthy nutrition and physical fitness through the utilization of national campaigns such as National Nutrition Month and other nutrition promotion efforts for patients and employees. Facilitates community classes such as: Fun with Cooking, Lifestyle Management, Diabetes support groups, Diabetes self-management group, and other chronic disease management classes as needed. Works with One Community Health''s Human Resources Department to provide Employee Wellness Program education, and activities. Participates in the community garden/edible landscape project. Community Partnerships/Collaboration/TeamworkWorks with other community agencies to meet the needs patients and complete projects as approved by supervisor. Helps with the fun with cooking classes and at the cooking classes at the Sacramento co-op.Works with undergraduate university students and post graduate dietetic interns as approved by management. May assist volunteers and interns working on-site. Provides nutrition/health education, assessments and/or presentations at community events as requested. Monitoring and EvaluationImplements monitoring and evaluation tools to assure effectiveness of programs, meet funding source requirements, and requirements of Patient Centered Medical Home and CQI team.Continuous Quality Improvement: Participates in identification and implementation of systems for incident/problem identification and appropriate resolution and feedback process. Implements recommend improvements to process in all areas of responsibility.This position is responsible for reporting suspicion of child, adult (including spouses), elder, and dependent adult abuse as outlined in Penal Code, Section 11166, Section 15630, and Section 1160.Education/Experience RequiredBachelor of Science Degree or Masters of Science in Nutrition or DieteticsCompleted Dietetic Internship and passed the CDR exam to be a registered dietitian.Registered, or is eligible to be registered by the Commission on Dietetic Registration, the credentialing agency of the American Dietetic Association, with one year of experience in the field of perinatal nutrition.CPR certificationRegistrations and certifications required for this position must be kept current, valid, and in good standing with the presiding entity.Our BenefitsPlease click here for information on the comprehensive benefits we provide.
