This is a full-time, day position (40 hours/week).Please note:this position is Monday through Friday, 8:00 am to 4:30 pm with some weekend and holiday requirements.Responsibilities:Manages entire hospitalization of a caseload of patients from preadmission through post discharge.Coordinates care with physicians, nursing staff, and other health professionals to ensure that patient outcomes are achieved within the established time frame and with maximum efficiency in resource utilization.Conducts initial and ongoing assessments, initiates clinical care protocols, determines and manages outcomes, ensures continuity of care through discharge planning, utilization of resources, and analysis of positive and negative variances.Qualifications:Registered Nurse License in NJ requiredBachelors of Science in Nursing requiredThree or more years working in an acute care/trauma hospital setting/insurance/home care, case management experience preferredCCM certification preferredRWJBarnabas Health is an Equal Opportunity Employer
