DescriptionName: TBD Unit/Dept.: Crossroads Resource CenterJob Title: Case Manager FLSA Status: FT/NEReports to: Lead Case Manager Schedule: 40 hrs./wk. TBDSupervises: N/A Start Date: TBDThis is a full-time, regular position that starts at $22.50 per hour and is eligible for but not limited to medical, vision, dental, life and disability benefits as well as vacation and sick time-off accruals.Function:The Case Manager position of The Salvation Armys Housing Now program involves working with homeless men. The Case Manager is part of a team that includes the Crossroads Shelter Programs Director, Lead Case Manager, housing navigators and Interns. The case managers primary goal is to cultivate relationships with clients, landlords, establish case management goals and tasks to prepare client for housing, link individuals with housing, managing financial assistance funds (application fees, security deposits, first and last month''s rent) and implementing all policy and procedures. Strong inter-personal engagement skills, motivating skills, time management skills and the ability to work with a very diverse population is required for this position.Duties and Responsibilities: Participant case management outreach and relationship building.Develop rapport and communication with both community resources and participants experiencing homelessness.Regular coordination with Program Director, Program Manager, and Case Managers, including attending collaborative working group meetings.Conduct client-driven housing searches based on the individual clients'' needs.Accompany clients in housing search and outreach to available housing options when needed.Act as client advocate in securing appropriate housing.Collect all necessary documentation needed to support assistance.Prepare request for financial assistance and submit all required documentation to supervisor for approval.Identify housing options for program participants and support each households housing focused case management plan using motivational interviewing and trauma informed care.Extensive record keeping, data entry and documentation in HMIS. (Homeless Management Information System)Maintain comprehensive case management files and statistics required by funding sources.Keep current with landlord laws, ordinances and regulations that impact housing.Attend trainings, workshops, seminars, community events and conferences related to this position.Responsible for maintaining confidential, professional boundaries with all clients.Work with volunteers, interns, staff and outside agencies to facilitate program activities.Performs other duties as required. Qualification and Education Requirements:Bachelors degree in human services, social work or related field is required.Experience in the community resources linkages in the Denver Metro Area with homelessness prevention preferred.Two years of experience providing housing services to highly vulnerable populations preferred.Case management experience preferred.Be supportive of The Salvation Armys mission. Ability to reflect and model the high standards of our organization as one of the worlds most distinguished human services charitable organizations.Knowledge, Skills and Abilities Required:Strong knowledge of homeless services and resources.Demonstrated knowledge and experience with Harm Reduction, Motivational Interviewing, Critical Time Intervention, and Housing First.Must have experience identifying housing opportunities.Understanding of Section 8, affordable housing, Rapid Re-housing, HUD preferred and housing vouchers and understanding of issues around addictions and homelessness, including employment, job skills, and financial health are essential.Must be highly motivated and a self-starter.The ability to communicate with and relate to a diverse group of people including clients, community, and other staff.Must have excellent organizational skills and the capability to work in a fast-paced environment.
