Find out: Full-Time, Monday-Friday, Schedule can be flexibleHighlights of the position:Demonstrates kindness, awareness, excellent communication skills, and flexibility to support primary care teams in an outpatient setting. Collaborates with leadership and Operations teams to help to design, champion and implement clinical policies and procedures that promote person-centered, integrated primary care.Performs RN Support functions, including but not limited to:Reviews desktop within Care Team for urgent/time sensitives issues, requests, labs, and more.Triages and returns patient phone calls, relaying instructions and addressing phone notes at the request of the PCP.Performs Rx refills as per existing Rx Refill standing orders.Supports quality efforts by reviewing quality reports and collaborating with Quality Business Partners on areas identified as needing improvement.Maintains timely, accurate, and thorough documentation of all patient interactions whether in person, remote, or telephone.Works collaboratively across teams, prioritizing trusting relationships while also being cognizant of always functioning at the top of license.Is flexible in adapting to the needs of the department, stepping in to provide coverage to support other Care Teams as needed.Collaborates within teams to ensure RN Practice Support services are patient-centered and mission-focused.
