LPN Care Coordinator- Home Health Private Duty

The Visiting Nurse Association of Florida (VNA) is a 45 year old home health agency that is growing and looking to hire an LPN Case Coordinator for our Stuart Branch.As an LPN Case Coordinator you will assistant with the clinical manager workflow. Act as a liaison for VA nurse case managers, discharge planners, and community care planners in order to coordinate care for the VA patients in their homes. You will work with our field staff in solving any clinical, medication, or supply issues and report that information back to the VA care team. Obtain orders and authorization for all VA skilled patients and request any additional services. Review monthly expiring orders/ authorizations and request extension or discharge. Write updated wound care orders and fax lab reports to providers. Manage the Qliq software application, cover for the per visit scheduler when needed and track insurance authorizations.
