Monitors progress to their physical and behavioraloutcomes. Implements patient assessments. Specialized Nursing care of patientswhich may include care plan development, quality assurance and makesrecommendations to the Nurse Administrator. Plans supportive actions to promote optimum health using family centered model ofcare. Ongoing evaluation of the plan of care. Follows up on patient needs andoutcomes. Collaborates with personnel and team to meet patient objectives. Adheres to quality standards. Audits records. Advocates for patients and theirfamilies while coordinating effective health care services. Facilitates transitionsappropriately. Conducts Outreach and attends meetings as appropriate. Provides health teachingto patients and families. Develops patient education programs and counsels''families. Collaborates with patients, their families and others to maximize patient resourcesand facilitate decision making process. Participates as an interdisciplinary teammember Maintains compliance with personnel requirements and follows personnelguidelines. Notifies managers and Human Resource Liaison appropriately on humanresource related issues.
