Our team of psychiatrists and psychiatric nurse practitioners provide comprehensive psychiatric evaluations, medication management, and psychotherapy to the residents of our long-term communities. Our practitioners provide these services using an interdisciplinary, collaborative team approach to ensure the combination of therapy techniques and medication are used for the best patient outcomes. We are hiring a part-time Psychiatric Mental Health Nurse Practitioner in Indianapolis, IN to work with patients with the goal of reducing polypharmacy when appropriate, managing psychiatric medications, and implementing Medicare compliant gradual dose reduction strategies for psychotropic medications.Our PMHNP''s provide direct patient care; stabilize patients, minimize complications, provide physical and psychological support, facilitate restoration of maximal health potential, and evaluate health risk factors. They perform diagnostic assessments, acquire health histories, perform examinations, prescribe and manage psychiatric medications, integrate psychotherapy when appropriate, and manage patients in a nursing facility or assisted living environment. Specific duties include:• Appropriately document medical interventions, including initial patient history, physical examination, review of current medications, labs and other relevant information from the patient''s medical chart, clinical progress and interventions, and collaboration with other applicable healthcare providers. • Obtain and accurately document all relevant health history, with an emphasis on mental health history.• Perform comprehensive mental health assessment and psychiatric evaluation, to include: mental status, current/past history of violence, suicidal or self-harm behavior, substance use, trauma, sexual behaviors, and social/developmental history.
