Location: Nashoba Valley Medical CenterPosted Date: 7/18/2022JOB SUMMARYIn collaboration with the treatment team and Quality Management, ensures that, from admissions to discharge, a patient receives appropriate, timely and high quality care in the most suitable setting while addressing an individual''s needs to achieve quality cost-effective outcomes. This includes utilization review/cost containment and discharge planning responsibilities.KEY RESPONSIBILITIES:Provides comprehensive clinical resource management for patients.Coordinates patient care to improve quality of care through the efficient use of resources and thereby enhancing quality, cost-effective outcomes.Works closely with the patient, nursing and physician staff to restore health to the patient.REQUIRED KNOWLEDGE & SKILLS:Integrated clinical nursing experience.One to two years of related clinical, continuing care, UR or Case Management experience preferred.EDUCATION/EXPERIENCE/LICENSURE/TECHNICAL/OTHER: Graduate of an accredited School of Nursing.Current Massachusetts nursing license.Bachelor''s degree preferred.Certified case manager or completion of a certificate program preferred.Additional InformationSteward Health Care is an Equal Employment Opportunity (EEO) employer.
