Helps patients to respond constructively to treatment and assist in adjustment leading to and following discharge. Interprets psychiatric treatment to patients family and helps to reduce fear and other attitudes obstructing acceptance of psychiatric care and continuation of treatment. Serves as link between patient, psychiatric agency, and community. Works directly in treatment relationship with patients, individually or in groups, in consultation with Psychiatrist. Consults and coordinates care of patient with physician, program coordinator and agencies. Assesses treatment planning and progress. Prepares treatment plan and goals for each individual patient/resident. Provides crisis, assessments and intervention. Maintains pertinent patient data in medical record in accordance with state, JCAHO and Mercy Health System guidelines. Offers in-service education programs and functions as a resource to other staff and the community as needed. Participates in development and selection of patient education materials and services. Participates in performance improvement activities. Education and Experience Masters degree in Behavioral Health field and appropriate level of field experience and coursework. Certifications/Licensure Licensed in one of the three state licenses (LMFT, LCSW, LPC) or a higher designation. While performing the duties of this job, the employee is frequently required to sit, talk and hear. The employee is occasionally required to stand; walk; use hands to handle, finger, and feel; and reach. The employee must exert up to 10 pounds of force occasionally. Specific vision abilities required by this job include frequent close vision and occasional ability to adjust focus.
