RN or RN Referral Specialist

At Central Maine Healthcare our team members are committed to providing exceptional care and experiences for our community and for each other every day. 10AM-10PM - Full TimeEssential Duties:Responsible for facilitating patient flow to and from CMH and external agencies or organizations.Reviewing requested care needs while simultaneously balancing bed requests with priority of need.Obtain patient information critical to determining appropriate CMH provider, medical necessity and level of care.Coordinating phone call between the referring and receiving providers.Assisting referring facilities in selecting appropriate mode of transportation.Prioritize calls to assure timely access to services at Central Maine Medical Center.Maintains a working knowledge of current CONNECT Transfer Center directives and Standard Operating Procedures (SOPs).Utilizes the screening guidelines to connect the appropriate providers.Works with providers to achieve discussion or acceptance and the most appropriate bed placement for patients.Obtains clinical updates on patients awaiting placement in the region.Works in coordination with staff from the nursing supervisor and nursing leadership to identify appropriate initial bed placement.Collaborates with Nursing supervision and providers on flow prioritization.Participate in training/orientation of new employees to CONNECT.Performs other duties as assigned or required.Education and Experience:Associates degree required, BSN preferred.Valid State Nursing License required.Minimum 3 years of clinical RN acute care experience preferred.ED or ICU experience preferredKnowledge, Skills and Abilities:Ability to engage patients and team members utilizing the CMH Experience Standards: I am creating a warming, caring, and non-judgmental environment I am actively listening and seeking information I am honest, truthful, and consistent I am respectful, treating all individuals with dignity and empathy I am serving as a role model, taking both initiative and ownership when appropriate I am working collaboratively and demonstrating teamwork I am resilient and adapt to change in positive ways Ability to communicate clearly.Ability to handle stressful situations calmly and deal with difficult customer(s) professionally.Excellent problem-solving skills.Demonstrates accountability for own decisions and behaviors, exercises critical thinking, clinical rationale and ethical decision-making in all aspects of professional practice.Nursing framework and professional practice.If you are passionate about making a difference and are looking for your next great career opportunity, we look forward to reviewing your application!
