Job DescriptionJob descriptionCollaborates with the LAAO partners and providers to support the network of resources focused on care across the continuum for patients with atrial fibrillation requiring left atrial appendage occlusion device placement. Serves as a liaison between respective partners, campus specific teams, patients, and the diverse communities we serve. Responsible for facilitating transition of care, scheduling initial consults, testing, and referrals. Participates in initiatives that elevate the care for this patient population and stakeholders. Job Description: The LAAO Nurse Navigator will have a unique position in the structural heart disease specialty that will act as a subject matter expert.Duties include: represent a primary point of contact for patients as well as SLCC cardiologists facilitate comprehensive transition of care ensure appropriate and timely workflow of diagnostic tests and subspecialty management offer communication and education to the patientcollaborate with the healthcare team to respond to the needs of outside referring hospitals, clinics, and cardiologists participate in the growth and development of the LAAO Program through internal education and community outreach partner with structural team in initiatives that elevate the patient care experienceScreening for appropriateness of referralsCreate a process for coordination of care and follow up for the LAAO patient population and maintain that processAct as an integral part of the shared decision-making process for LAAO patientsJob RequirementsApplicable Experience: 2 yearsBasic Life Support - American Heart Association or Red Cross, Registered Nurse (MO) - National Council of State Boards of Nursing (NURSYS)Bachelor''s Degree (Required)Skills: Detail oriented with excellent communication and follow through, demonstrates strength as a change agent and collaborative partner in diverse environments, displays strong analytical qualities, effectively initiates solutions, seeks staff input for decision making, engaging teaching skills, elicits confidence and leadership traits.Job RequirementsApplicable Experience:1 yearBasic Life Support - American Heart Association or Red Cross, Registered Nurse - VariousBachelor''s Degree - Science in NursingJob DetailsFull TimeDay (United States of America)The best place to get care.
