Essential Functions:Because HCF exists to care for our residents, regular attendance and punctuality are essential to provide quality careLeadership: Ability to take action and achieve results through others Commitment to HCF mission, values and philosophy and Service Excellence InitiativeMust be able to complete performance requirements independently while using good judgment as well as have the ability to harmoniously work with othersMust be able to read, write and understand the English languagePosition Responsibilities:Must follow HCF Policies and ProceduresMust meet job related competencies as outlined in the Skills Competency ManualPre Admission: Conducts on-site assessments, verifies payer source, completes pre admission screening form, pharmacy and med surge quotesAdmission: Review and initiate orders, communicate with care community personnel, greet resident and familyCoordinate residents MDS''s and Care ConferencesParticipates in the QA process and ''Nurse On-call'' programOn-going communication regarding resident goals, progress and discharge planningAssumes responsibility for maintaining his/her own professional competenceOversees skilled residents care and works closely with the Office ManagerPerform other duties as assignedKnowledge, Skills and abilities:Graduation from an accredited school of Nursing. Current license as a Registered Nurse with the State Board of Nursing. CPR certification recommended.EOE m/f/disability/vet
