We are conveniently located near Interstate 494 and Highway 52 and public transportation.We are looking for an RN Case Manager/Registered Nurse. This nursing role would be responsible is responsible for ensuring that the safety and health needs of the senior assisted living and memory care residents are met with support from staff such as resident assistant(support role).Nursing position schedule includes;64 hours in two weeksEvening shifts Every third weekend rotation (possible Day shifts) and holiday rotationBenefit eligible (paid time off, medical, dental, matching retirement monies, tuition reimbursment) DescriptionMaintains current assessment and documentation using progress notes of resident health statusImplements new and updates current resident care planCoordinates resident health services with internal/external providers and getting orders as neededManages and oversees timely and accurate medication and treatment administrationActs as support to Director of Health Services to fill in as RN on call during scheduled timesMonitors work and performance of licensed and unlicensed staffQualificationsRequired:Current MN RN license or by start dateAHA BLS for Healthcare Providers or Red Cross CPR for Professional RescuersPreferred:Must be comfortable using Microsoft windows and electronic charting systemsWork experience in a patient support role, LPN or RNGood written and verbal communication skills to interact with residents, families and physicians or providersAbility to work with and have sensitivity to the needs of the geriatric population
