You will have the satisfaction of knowing you are ensuring the patient receives optimal care, by assessing, planning, implementing, re-evaluating and revising care and coordinating resources. Among other responsibilities, you will update and maintain patient records and implement orders for medications and pletes the initial psycho-social assessment and develops a plan of care for social servicesAssesses the social and emotional factors in order to estimate the patient''s capacity and potential to cope with problems of daily livingProvides social work intervention through counseling and education for patients to maximize coping capacityIdentifies and coordinates community resources for the patientInstructs the patient and family to understand, accept and comply with medical recommendationsProvides services that promote and restore the patient to optimum social and emotional adjustment within capacity of the patientAssists the physician and other disciplines in understanding the social/emotional factors related to the patient''s health problemsQualifications• Bachelor of Science degree in Social Work• Licensed Bachelor Social Worker / LBSW certification• Previous home health, hospice, skilled nursing, rehab facility, hospital, mental health clinic, or child welfare setting experience preferred• Familiarity with medical software and equipment - MatrixCare/Wellsky a plus• Participation in multi-disciplinary patient case conferences, staff meetings and in-service education programsAbout our Line of BusinessAdoration Home Health and Hospice provides quality and compassionate services in the comfort of home, providing support for patients, families and caregivers in their time of need.
