Job DescriptionPosition Summary: The Assistant Director of Community Services Nursing manages and oversees the nursing services provided to the residents of the IRA in accordance with the OPWDD regulations as well as Vanderheyden''s Policies and Procedures. Primary Duties & Essential Functions: Health Care Provisions/Oversight Acts as primary liaison with hospital as needed when individual is hospitalized. Ensure discharge meeting and proper follow up complete Participate in new employee orientation as assigned. Ensure and assist with scheduling appointments and maintaining appointments. Assist with establishing providers for individuals. Completes monthly reports. Maintain and reports quarterly to Director and VP medication reliance among individuals. Responsible for the provision of health services as defined in the job description for IRA Residence Nurse. Ensures the health care needs of all individuals are met and provided for in accordance with the Nurse Practice Act of NYS. Develop and maintain systems that assure that all IRA Programs are operating within regulatory guidelines in accordance with OPWDD. Oversee with training of OPWDD medication class ensuring along with completes medication practicums for the IRA Program . Provides for individuals and staff educational training related to health concerns. Schedule and assist pharmacist with semi-annual Pharmacy Medication Reviews. All RN nurses will participate in the Nursing On-Call System after 3 months on job. Audit on a monthly basis to meet OPWDD compliance. 2 Charts per site turn into Director of Health Services Ensures nursing oversight to Day hab Program and ensures regulatory compliance with in correlation with Director of Health Services Ensures medication administration certification compliance across IRA and Day hab Programs and complete monthly. Other duties as assigned by Director or VP. Supervision of Staff Provides regular weekly supervision and consultation with the IRA nursing staff in regards to residents health care management. Conducts employee performance appraisals in a timely manner. Direct and supervises nursing staff in regards to health record of each absence of IRA residence nurse will assume those duties as related to the needs of program. In absence of Director of Health Services will assume those duties as related to IRA programs. Any LPN responsibilities/assessment will need to be reviewed and approved by Assistant Nursing Director. Quality of WorkWill monitor all documentation in OPWDD program to verify IRA residence nurse are meeting all regulations Will coordinate and assist IRA nurses as needed with individual health needs. Will assess individuals to make sure medical needs are being met in a timely manner. Will have adequate knowledge of OPWDD regulations to assist and educate staff. Will assist with educating staff as needed regarding developmental disabilities. Assist nurses at treatment team meeting to address medical issues based on individuals health care needs. Interpersonal Communications Provide training and education to staff when needed. Obtain information if needed to provide education to collateral staff. To be objective observer. Adhere to and model agency personnel policies, operating procedures and philosophy in accordance with agency mission Maintain agency confidentiality in accordance with ANA Code of Nursing and Ethic and agency policies/procedures. Ensure open communication with Director of Programs Initiative Participation in agency committees including informed consent and Human Rights Be willing and able to take on new challenges and assist IRA nurse to adapt to program growth and change. Treatment/Collaboration To be able to work effectively with house supervisors, clinical staff, school/program personnel, collateral medical personnel during medical emergency. Maintain adequate contact with staff and nurse regarding individual progress. Attend Treatment Teams as needed to assist program nurse/house manager to educate staff and assist if needed to develop strategies in response to health care needs of individuals. Promotes and supports cultural diversity and competence and is strength based.; Planning and Organizational To be able to effectively manage time to provide support to staff spread out through the community To be flexible to meet needs of staff performing medication practicum during their work schedule to meet OPWDD regulations. RequirementsRequired Education , Knowledge and Skills: Graduate of an accredited school of professional nursing NYS license as a registered nurse Demonstrated competence as an RN Minimum 5 years-experience in developmental disabilities or psychiatric nursing preferred Previous supervisory experience preferred Valid NYS drivers license with a clean MVR Complete SCIP certification OPWDD nurse training if no OPWDD experience Travel between locations required Abilities and Working Conditions: Must be available to work a minimum 40 hour work week Must be able to life 25 pounds Must be able to stand for moderate periods of time Must have a valid driver''s license Must be able to perform physical interventions and maintains certification.
