About the position The Clinical Pharmacist Specialist Infectious Disease (ID) is responsible for development, implementation, and documentation of the antimicrobial stewardship program. This pharmacist will coordinate the pharmacokinetics service according to the most recent guidelines and recommendations published in pharmacy and medical literature.The pharmacist will assist other hospital personnel in the development of clinical pathways, protocols, polices and procedures and provide continuous education to students, residents, pharmacists, and other health care professionals. Requirements Degree Pharm.D., PGY 1 Residency Program. Two (2) years of experience in inpatient pharmacy hospital/clinical capacity. Currently licensed as a Pharmacist in Louisiana. Preferred candidates will have completed a PGY 2 Residency Program in Infectious Disease and/or obtained Board Certified Pharmacotherapy (BCPS) Responsibilities Participate in provision of patient-focused services. Provide pharmaceutical care to patients receiving antimicrobial therapy. Evaluates antimicrobial regimen for appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions. Switch patients to IV to PO therapy as quickly as feasible. Discuss medication order changes/clarifications with the prescribers, document any changes in patient and pharmacy records, and inform others of medication order changes. Provides patient consultations as requested for pharmacokinetics, antibiotic therapy choice, drug informationetc. Detect, monitor, document, and report adverse drug reactions and medications errors. Develop, monitor and adjust antimicrobial stewardship program (ASP). Collaborate with infectious disease physicians, pharmacy clinical directors/managers, infection prevention, nursing, clinical laboratory staff and other healthcare professionals. While also developing interventional strategies to maximize appropriate antibiotic utilization and improvement in patient outcomes included but not limited to antibiotic guidelines, clinical pathways, restriction criteria, policy development, new laboratory methods and formulary management. Establish a means to report and monitor ongoing anti-infective utilization and identified metrics showing effectiveness of ASP program. Identify opportunities for improvement regarding anti-infective use, culture reporting and ASP communication, including but not limited to performing medication use evaluations (MUEs or QA) and conducting prospective audits. Actively participates on the infection control committee, pharmacy and therapeutics committees and other appointed committees as needed. Perform formulary evaluation and antimicrobial class reviews for anti-infectives and present to P&T committee. Documentation of all activities/ interventions for cost savings and outcome purposes. Collaboration and ongoing Education of hospital and Pharmacy department Staff. Collaborates with hospital and pharmacy staff to insure timely resolution of problems and questions. Fosters team-work and takes responsibility to ensure delivery of high quality care. Provides ongoing education to hospital staff and pharmacists by developing educational material and competencies and providing in-services as needed. Awareness of regulatory standards: Demonstrates awareness and understanding of documenting information in the medical record, JCAHO standards and State Board of Pharmacy requirements. Demonstrates awareness and understanding of departmental and hospital policies, and health-system initiatives.
