Overview Dignity Health Sierra Nevada Memorial Hospital is a 104-bed not-for-profit hospital located in Grass Valley California. The hospital has been providing compassionate quality health care to residents and visitors of western Nevada County since 1958. As an affiliate of the nationally recognized Dignity Health system we ensure the highest standard of health care to our patients and have access to important regional resources throughout the system including the Dignity Health Heart & Vascular Institute the Dignity Health Neurological Institute of Northern California and the Dignity Health Cancer Institute of Greater Sacramento. With 787 employees 101 active medical staff and 21 Emergency Department beds Sierra Nevada Memorial Hospital continually implements and upgrades its technology and recruits employees who understand the vital importance of kindness and compassion in the healing process. Responsibilities The Registered Nurse is a professional who provides, plans, and directs care to individuals and families based on the nursing process in collaboration with the health care team. The RN does so while assuring a safe, quality and confidential care environment. Essential Functions: Assessment, teaching, planning and coordinating care including discharge needs, performance of RN-specific tasks, and providing team leadership to assure that care is accomplished effectively and efficiently. Care Team members focus primarily on their Essential Functions, but all members of the team perform other duties as their scope of practice allows in order to meet patient needs efficiently and effectively and in support of cohesive team functioning. Responsibilities 1 ASSESSMENT 1.1 Collects patient data based on diagnosis, age and/or developmental age and analyzes data collected. 2 PLANNING 2.1 Plans and prioritizes individualized care based on the assessment, utilizing nursing diagnosis, including teaching and discharge needs of the patient and family, and sets outcome goals in coordination with other disciplines. 2.2 Ensures that the patient/family participates in the plan of care as able, and that plan of care addresses age-specific and cultural needs. 3 INTERVENTION 3.1 Implements appropriate standards based on assessment. 3.2 Prioritizes and organizes care within the allotted time frames. 3.3 Coordinates discharge steps or ensures appropriate referral to address timely, appropriate discharge needs. 4 EVALUATION 4.1 Evaluates patient''s response to care. 4.2 Modifies plan of care based on evaluation. 5 CLINICAL SKILLS 5.1 Demonstrates competency and efficiency in psychomotor skills generic to scope of practice and in those required in specific area of care. 5.2 Responds effectively in emergency situations. 5.3 Respects patients'' rights. 6 LEADERSHIP 6.1 Directs care team members and ancillary staff in implementing patient care and delegates appropriately. 6.2 Demonstrates effective problem identification and problem-solving methods. 6.3 Plans staffing based on individual patient need as identified by acuity and/or patient priority and collects data to justify variances. 6.4 Directs or modifies bed placement activities and unit team assignments based on changes in patient population. 7 COMMUNICATION 7.1 Demonstrates effective communication skills using active listening, giving/seeking/accepting feedback, and utilizing appropriate chain of command. 7.2 Ensures that patient and family needs are communicated to the health care team. 7.3 Performs timely, concise, and accurate documentation duties. 7.4 Protects the confidentiality of patient information. 7.5 Uses professional and therapeutic communication methods that incorporate appropriate psycho-social, cultural, age and gender boundaries. 8 WORKING RELATIONSHIPS 8.1 Formulates and maintains effective and respectful relationships with all health care providers, patients, and families/significant others. 8.2 Works collaboratively with all health care providers determine work flow distribution. 8.3 Provides input to peer/team review. 8.4 Provides input to recommendations for new hires to the care team as required. 9 ENVIRONMENT OF CARE 9.1 Maintains emergency equipment/supplies and safe environment to ensure readiness for urgent and emergent patient needs. 10 EDUCATION 10.1 Identifies patient/family/significant others learning preferences, abilities and needs and implements appropriate measures to meet them. 10.2 Actively participates in staff development activities for team members and precepts new employees and students. 11 PROFESSIONAL DEVELOPMENT 11.1 Practices in accordance with structure standards, policies, protocols, procedures, standards of care and evidenced based practice. 11.2 Practices in accordance with California Nurse Practice Act and regulatory standards. 11.3 Participates in and remains current in decisions and information affecting care area through attendance at a minimum of 50% of staff meetings, inservices, classes and completion of required communications. 11.4 Demonstrates self-directed learning and participation in continuing education to meet own professional development. 11.5 Seeks validation of knowledge when appropriate. 12 QUALITY IMPROVEMENT 12.1 Participates in care area/unit quality improvement activities. 12.2 Participates in the development of standards and improvement efforts aimed at efficiency and quality care. Qualifications REQUIRES: Telemetry experience required (Exception: when staffing is able to accommodate training, management will consider RNs with non-Telemetry experience). Current California State RN license or interim permit Current BLS Healthcare Provider Current ACLS Accurate assessment of cardiac rhythms with demonstrated understanding of appropriate interventions and medications. Accurate assessment of cardiac complications with demonstrated understanding of appropriate interventions. Accurate interpretation and use of unit-specific equipment.
