Clinical PharmacistTyndall AFB, FLPosition Duties and Responsibilities:Work directly with the Family Health Clinic as the Primary Health/Medical Home Clinical Pharmacist in Family Health or in the Pharmacy. Clinical Pharmacists will consult with other healthcare providers (e.g., physicians, dietitians, nurses, physical therapists, etc.) about patient treatment needs or options. Pharmacy Flight will maintain administrative oversight and control while coordinating utilization based on the above-mentioned specialties, 325th MDG mission needs, and operations tempo. Conduct and coordinate clinical investigations and research (consistent with other healthcare professionals) approved by a local or regional Investigational Review Board (IRB) and participating in outcome studies generated by the department of pharmacy and approved by the P&T Committee.Perform a full range of pharmacist procedures.Deliver comprehensive pharmaceutical services within the personnel, supply, and equipment capabilities of the facility.Counsel patients regarding appropriate use of medications.Review patient profiles at the time of dispensing in order to closely monitor medication therapy.Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age specific dosage, and other pertinent data are appropriate for individual patients. Intervention with providers shall be made for noted discrepancy of above listed pound medications as required.Ensure accuracy of all pharmaceutical products prepared or processed by technical staff.Work closely with clinical staff in developing patient wellness programs such as smoking cessation programs and asthma classes.Provide medication information and medication therapy guidelines to the clinical staff as required.Will provide training to other staff members as required.Will prepare and provide reports for the Pharmacy and Therapeutics Committee as required.The clinical pharmacist may be utilized as a staff pharmacist as mission dictates but will spend no more than 10% of their time in this coordination with pharmacy and clinic leadership, every effort must be made to meet or exceed Defense Health Agency metrics regarding clinical pharmacy and positive patient outcomes. i.e. appointments per week and assisting deployment focused initiatives to increase medical readiness.Minimum Qualifications: A Doctoral degree in Pharmacy from a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE).18 months experience within the last 24 months practicing as a clinical pharmacist. i.e. pharmacokinetic monitoring, physical examinations of mild to moderate complexity, clinical rounding, ambulatory or inpatient residency.Board certification in one or more of the pharmacy specialties recognized by the Board of Pharmaceutical Specialties.A current, valid, unrestricted license to practice Pharmacy in any one of the fifty States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.Must be a US Citizen.Potomac Healthcare Solutions offers the following benefits:Medical, Dental and Vision Insurance Health Savings AccountEmployer Paid - Basic Life, Accidental Death & Dismemberment and Short-Term Disability Long Term Disability401(k) Retirement Savings Plan Paid Time Off10 Federal Holidays Continuing EducationEmployee Assistance ProgramWellness Incentives and MoreAbout Potomac Healthcare Solutions: Potomac Healthcare Solutions is an award-winning, dynamic, and rapidly growing healthcare services company with a proven background in providing world-class health services for both government and commercial organizations.
