The passionate OT will perform a thorough evaluation of patients assigned to him/her, establishes goals and plans of treatment to return the patient with disease, injury or disability to maximum level of function as outlined in the Rehab Department Procedures/Operational Guidelines Manual. The OT will delegate responsibilities and supervise supportive personnel, assist in administrative tasks, and participate in educational, orientation and performance improvement programs. Master''s in Occupational Therapy preferred. One year Occupational Therapy experience preferred Licensed as an Occupational Therapist by the Virginia Department of Health Professions required American Heart Association BLS Certification requiredResponsibilities Uses current evidence, patient values, and personal experiences in making clinical decisions Able to clearly articulate the role of their professional discipline in the acute care setting to patients and colleagues Reviews pertinent medical records and conducts a patient interview Performs a systems review to determine the need for outside referral or for therapy services Selects appropriate examination components based on the initial systems screen in order to define the patients therapy needs Synthesizes data, interprets findings, diagnoses dysfunction, determines prognosis Provides patient-specific interventions within scope of practice Assesses outcomes of interventions provided and adapts techniques as needed, based on a patients cognition, culture, communication needs, learning style and/or physical response Provides a clear and concise home exercise program at initial evaluation and updates as patient progresses. Provides education to the patient and/or family to deepen understanding and support the plan of care. Chooses mode of education based on patient specific variables that affect learning. Demonstrates ability to identify and address barriers to home discharge in acute care Advanced practice includes a combination of advanced and distinctly increased clinical and analytical skills, knowledge, clinical reasoning, attitudes and experience. This may occur within a specialty or in generalist practice. Specialty skills for OT include, but are not limited to the following: splinting, taping techniques, use of specialty equipment, care of medically complex and critically ill patients Writes clear and defensible documentation consistent with facility specific standards that articulates clinical decision making and skill of the therapist. Documentation and overall chart management stand up to internal and external audits Minimizes use of abbreviations and utilizes only those approved by the hospital. Writes notes that are clear enough to be understood and treatment reproduced when appropriate by another therapist Notes significant clinical data, investigates further, communicates with physician or appropriate non-physician provider accordingly and documents communication correctly. Addresses patient/family needs or concerns Creates goals that are specific, measurable, achievable, relevant, time-bound, and functional Justifies need for skilled care with impairments listed, pre/post treatment measurements, detailed reassessments that demonstrate medical necessity with revisions to plan of care and/or goals when appropriate Conveys appropriate prioritization of elements of the patients plan of care Completes discharge summaries at time of discharge following department guidelines and includes recommendations to address patient discharge needs, including additional rehab services and equipment. Works under valid orders and/or direct access guidelines, as applicable Assigns appropriate diagnostic codes Accurately bills charges without error Assigns proper complexity codes for evaluations Consistently documents reasons patients are not seen as scheduled/planned. Works collaboratively as part of interdisciplinary team for discharge planning assessing the expectations and desires of all stakeholders with an understanding of regulations imposed by the healthcare systems and payers. Submits productivity worksheet in a timely manner Meets department standard for productivity Accepts student placement when requested by Student Coordinator Openly communicates with Student Coordinator throughout clinical experience, seeking consultation in a timely manner for students with ongoing challenges or who require remedial intervention. Models professionalism, safety, collaboration, effective communication, leadership, evidence-based practice, positivity, and acts to promote the student as a colleague to others. Adapts teaching strategies to accommodate students learning needs. Follows guidelines from the students school for performance expectations and completes required evaluations on time. Provides positive and constructive feedback to the student in real time when possible, and in a summation during weekly goal setting meetings. Adheres to facility, state, federal, and professional supervisory and documentation requirements. Draws out the patients story and collaboratively develops an individualized care plan Creatively negotiates and works with patients to improve customer service and patient ownership of their role in therapy Follows clinical practice guidelines, when available Incorporates tests/measures validated for the patient population and sets goals using minimal detectable change, minimal clinically important difference or established cut scores, when available Suggests, implements, documents and participates in hospital, department and/or team quality improvement activities or program development. Sets and meets deadlines and commitments. Limits co-treating to allow patient to obtain more individualized sessions Provides treatment with evaluation when appropriate Assigns appropriate frequencies to meet patient needs with attention to hospital resources Properly prioritizes caseload per department procedure when staffing is limited Monitors patient status in cases of medical instability, transfer to another unit, or other changes in status. Communicates with the healthcare team to determine when patient can participate in therapy.
