YOUR CONTRIBUTION TO OUR SUCCESS!($10K Sign On Bonus)As an AHF Care Management Program Regional Professional Resource Manager, you will work to create connections and provide solutions to the Community Health Worker (CHW) staff as they assist our vulnerable patient population and their AHF Healthcare Center healthcare providers. As a Regional RN Resource Manager you will be called upon to utilize your many unique talents to assist and direct the CHW staff as they help patients navigate the healthcare and social service systems in various and challenging ways. As a Care Management professional, you will have the unique ability to bring information and support to AHF patients, CHW staff with the goal of achieving Medical Provider and patient healthcare goals. e.g., providing access to higher acuity care management services currently established or available to the patient. Provide education and direction on professional resources and working with the CHW staff so that the goals of the programo Viral Load Suppression for 90% of patients enrolled in NCM. o Decrease the number of patients on the 6-12-month report by 33%o Successfully link 50% of patients who are moving to another area where AHF services are located are understood and achieved. As the Regional RN Resource Manager, you will support your CHW staff by providing insight, education and care management direction concerning patient and community barriers to care. You will direct patient care management needs to the appropriate services and programs, monitor metric reports so that the performance of the program is known at all times. You will report directly to the Director, provide current program performance, trending, successes and challenges. You will meet with your CHW staff (1:1) on a monthly basis and understand the program process from the frontline agent perspective. You will provide oversight of this innovative approach to whole person care addressing health and social needs, monitoring and collecting data, ensure processes, program and policies are followed and assist your CHW staff in resolving care management concerns, achieve patient goals and optimum patient outcomes and retention in care. You will convey the purpose and services of the Community Health Work to your patients, the HCC team and all of Care Management. You will coach and assist patients in understanding and accessing community services and healthcare system support while incorporating AHFs core values of patient first, respect for diversity, being nimble, fighting for whats right and valuing your fellow employees and our AHF Mission: Cutting Edge Medicine and Advocacy, Regardless of Ability to Pay. As Regional RN Resource Manager, you will ensure Care Management works to keep members engaged in and understanding their healthcare, support your team as they provide coaching to patients so they may improve their health outcomes. As part of the Care Management Team, you will work with all patients of AHF, regardless of payer source, to provide support and access to services impacting their ability to achieve better health outcomes.
