***OFFERING A $5000 SIGN-ON BONUS*** Major Responsibilities: Provision of Patient care Staff Education and Development Patient/Family/Community Education Quality Improvement/Evidence based practice/Research Licensure: License to practice in Illinois or obtain license within 90 days of hire date. CPR Certification BCLS Education/Experience Required: Entry level Master''s/Doctorate Degree (or Bachelor''s degree prior to 2004) in Physical Therapy from an accredited school. Minimum 3-4 years in area of clinical expertise. Maintains continuing education requirements as mandated by professional standards. Successful experience with supervising senior level student and/or mentoring new staff/peers. Participates in research and/or provides continuing education programs beyond the level of the facility and/or acts as a chair or co-chair of a committee or task force. Knowledge, Skills & Abilities Required: Strong interpersonal and communication skills. Independence with providing advanced level of clinical skills and expertise. Prefer ability or previous experience in holding crucial conversations. Clinical specialist via Association (AOTA, APTA, ASHA) or holds specialty training that requires ongoing maintenance (i.e. hours, re-test) for renewal. Physical Requirements and Working Conditions: May be exposed to hazardous materials and life-threatening diseases • Direct contact with chemicals and body fluids Ability to assist, escort, transfer, and respond to patient needs for extended periods of time. Pushing, pulling lifting and transferring patients. Ability to perform heavy lifting (lifting 50 to 100 pounds occasionally and/or 25 to 50 pounds frequently and/or 10 to 20 pounds constantly.) • Ambulating with patients over a variety of surfaces with and without assistive devices • Services provided on weekends and holidays and during early evening hours This requirement supersedes any previous lifting requirement effective 08/01/2014. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis. This job description indicates the general nature and level of work expected of the incumbent.
