Job Description

The therapist plans and delivers outcome-effective clinical, case coordination, intervention, prevention, consultation and/or mental health treatment services to pediatrics and their parents, and the agencies that serve them. Services are provided both within this agency and in the community, in families'' homes, in schools, and other social service agencies necessitating transportation. Services are also provided via Telehealth and it is requried that therapist utilize this modality as needed. JOB DUTIES:• Demonstrate adherence to the PHP/SBHI Patient Experience philosophy, guidelines and tools.• Demonstrate strong ethics, honesty, and adherence to PHP/SBHI Code of Conduct and PHP/SBHI Compliance Program.• Schedule and complete direct face to face and/or remote assessments within a timely manner according to departmental standards and productivity expectations.• Provide other direct, face to face, and Telehealth clinical services to clients via individual, family, day tx/PH and group therapies according to departmental standards and productivity expectations.• Collaborate with referral source, internal team members and external service providers to ensure appropriate client care and continuity of care.• Provides case management and CPST services to coordinate clinical care in the form of face to face, telephone calls, report writing and case conference meetings.• Provides clinical services within a client-centered and strengths-based approach, consistent with the philosophies of YCATS and applicable accreditation organizations.• Complete all clinical documentation within set time frames per Program guidelines.• Demonstrate clinical knowledge and skills relevant to the clinical service and intervention method provided to both pediatric clients and their families.• Daily maintain scheduled appointments and other workload activities on the electronic scheduler according to SBHI standards.• Seek psychiatric/physician consultations regarding medical needs of clients. Maintain on-going communication with psychiatrists/physicians for the purpose of coordinating and evaluating client care.• Demonstrate knowledge of SBHI and YCATS clinical records and other paperwork standards, and apply competencies in both the provision and documentation of clinical records.• Demonstrate compliance with departmental standards for administration and reporting of outcome measures and documentation of utilization management of clinical care.• Demonstrate knowledge of regulatory standards and ensure that clinical records are maintained in compliance with these standards at all times.• Participate and complete and correct monthly Peer Reviews.• Ensure productivity standards are met consistently as applicable to departmental standards.• Demonstrate knowledge of Information Technology policies and procedures and apply them to the documentation of clinical care according to medical records and other departmental standards.• Demonstrate in daily clinical work the use of best practice techniques and plan clinical service delivery in accordance with a strengths-based treatment approach discussed with and approved by program director.• Participate in the research, strategic planning and implementation of activities to promote departmental services, including decentralized locations and new service initiatives.• Meet regularly with supervisor and as needed to review clinical care of clients according to licensing body, SBHI standards and administrative needs.• Consultation Services: Build effective relationships with external recipients of consultation services, and demonstrate a collaborative approach to consultation services.• Consultation Services: Present a positive image of YCATS and SBHI by demonstrating behaviors consistent with those of YCATS and SBHI. Engage customers/clients and facilitates access to therapeutic services as needed.• Respond within departmental timeframes to failed activities and alerts• Proactively provide communication regarding any problematic issues within CareLogic or other billing and documentation processes to supervisor.• Ensure releases of information are obtained for outside entities providing services to the client and initiate communication regarding client progress in other settings.• Maintain regular communication with client caretakers to facilitate treatment goals and to assess client and family''s commitment to treatment.
