A healthcare network in Westchester County is currently seeking a licensed Registered Nurse (RN) to join their staff as their new Case Manager. In this role, the Case Manager (RN) will be responsible for providing health related services to individuals who reside in the community residential program to insure the quality of their health status.ResponsibilitiesThe Case Manager (RN) will:Assist individual with activities of daily living, including toileting, showering, grooming, dressing, oral hygiene, meal programs, recreation, and specialized medical needs as indicated by the individual treatment planAdminister medication and respond to changes in the individual''s conditionParticipate with trans-disciplinary team in the development, implementation and evaluation of the individual''s program planActively implement programs designed by the trans-disciplinary team by following prescribed methodologyMaintains records and charts for each person by entering required information (goal progress, temperature, changes in behavior, etc.)Maintain medical equipment and suppliesSupervise records and charts for each resident, including entering required information (temps, weight, doctor orders, medication records, food and fluid intake etc.)Physically examine and prepare for presentation of individual or employee incidents or accidentsPerform all other duties, as assignedQualifications:2+ year of related Nursing experienceBachelor''s Degree in NursingNYS Registered Nurse (RN) licenseSolid assessment, clinical, and documentation skillsPatient orientedGreat interpersonal skillsExcellent communication skills (written and verbal)Strong attention to detailHighly organizedDesired Skills:3+ years of Case Management experience
