Hospice Rn Case Manager

Location: Oak Creek, WIDescription: Are you an RN looking for a Case Manager role? If so, then keep reading!Details:Case load is 12 patients in a 60-mile radius, seeing on average 4-6 per dayRNs need live in their territory (in or near Oak Creek, WI, )Monday to Friday 8 AM to 5 PM with weekend and holiday rotationsWhen working a Saturday and Sunday they will have the Thursday off prior to the weekend or Tuesday off after the weekendHave a large team and will be backup to the on-call teamCompensation:Base pay is commensurate and based on experienceBonuses for the following:Each Additional VisitsEach Death Pronunciation Each Admission Reimbursements:Mileage (for legal purposes, the initial daily commute to the first place of visit is not reimbursed)Tablet or phone (if applicable)Scrubs are providedVery strong benefits package!Qualifications:A passion for hospice care and the ability to build relationships with both patients and their family membersCurrent WI RN licensure and CPR certificationThe ability to work efficiently with little direct supervisionA valid driver''s license and an insured automobile that is in good working orderAcute care experience in an institutional setting or home care experience is preferred, but not required.
