We provide concierge care management, private duty nursing, and personal care assistance services to seniors.ResponsibilitiesThe RN Home Care Nurse position includes the following:Conducting assessments to identify problems, eligibility for assistance and need for servicesEnsuring in-home care delivery is in accordance with each client''s contractual program requirements and is high quality and complies with company -adopted evidence- based clinical practice guidelinesDeveloping a professional care plan from the assessment findings and in collaboration with the clients Physician to ensure the facilitation of the provision of the best possible careThorough and timely documentation of initial and ongoing assessments and outcomesCommunicate explanation of the care plan and all subsequent cost of all services to client and or families, Attorney or GuardiansAct as a liaison to families as well as with collateral clinicians, professionals & agencies.Effectively communicate with clients, families, Physicians, Attorneys, etc. and all members of the multidisciplinary team all findings, outcomes, plans that relate to the provision of client careWork effectively with all departments to ensure the best delivery of serviceParticipate in case conferences with other disciplines providing careObserves infection control practices and uses standard precautionsParticipates in quality management/performance improvement activitiesParticipation in On Call coverage (Weekdays = 5pm - 8:30am, Weekends = 24 hour shift)Required QualificationsRegistered Nurse with active unrestricted NY license2+ years of experience of in home care or care managementExperience working with the adult populationExceptional communication and interpersonal skills with the ability to quickly build rapportAbility to work with minimal supervision within the role and scopeAbility to use a variety of electronic information applications/software programs including electronic medical recordsEffective computer skills and experience with Microsoft Word, Outlook, and ExcelAbility to travel to client residences within Nassau CountyThis role is a part of Humana''s Driver Safety program and therefore requires and individual to have a valid state driver''s license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,00 limits.This role is considered patient facing and is part of Humana At Home''s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.We will requirefull COVID vaccination (for this job as we are ahealthcarecompany committed to putting health and safety first for our members, patients, associates and the communities we serve. This job requires close personal interaction with patients.
